Position Overview:
We are seeking a dedicated and skilled LTCF RAI Coordinator (LPN) to join our team. This role is essential in ensuring the accurate assessment and documentation of resident care needs in a long-term care facility. The ideal candidate will possess a strong background in nursing, particularly in long-term care settings, and will be responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring compliance with regulatory standards, and enhancing the quality of care for our residents.
Responsibilities:
- Coordinate the RAI process, including assessments, care plans, and documentation.
- Collaborate with interdisciplinary teams to ensure comprehensive resident evaluations.
- Conduct resident assessments and monitor changes in health status.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Provide training and support to staff on RAI processes and documentation standards.
- Participate in quality assurance activities to improve resident outcomes.
- Assist in developing individualized care plans based on assessment findings.
- Ensure compliance with state and federal regulations regarding resident assessments.
- Engage with residents and families to address concerns and enhance care experiences.
Qualifications:
- Current LPN license with the British Columbia College of Nurses & Midwives (BCCN&M)
- Minimum of three (3) years experience in senior care, preferably in a long term care setting.
- Experience with the use of PointClickCare.
- Currently LTCF RAI Course Completion.
