About the Role
HealthOPM is seeking compassionate and dedicated Personal Support Workers (PSWs) to join our growing team in Hamilton, ON. In this rewarding community-based, home-to-home role, you'll make a meaningful difference in the lives of seniors by supporting their daily routines, well-being, and independence.
Location: Hamilton, ON (Community Home-to-Home)
Shift: 5:00 PM 12:00 AM
Pay Rate: $20/hour
Position Type: Part-Time
Transportation: Must have a valid drivers license and reliable vehicle
Assist clients with activities of daily living (ADLs) and personal care
Provide emotional support and meaningful companionship
Observe, document, and report changes in health or behaviour
Support clients with mobility, hygiene, and nutritional needs
Collaborate with nurses and other healthcare professionals
Follow individualized care plans and maintain accurate charting
Adhere to infection prevention, safety, and confidentiality protocols
Valid PSW Certification (Ontario)
Recent Vulnerable Sector Check (within 12 months)
Clear 2-Step TB Test
Valid drivers license and reliable vehicle
Strong communication and interpersonal skills
Experience with dementia care and GPA/NVCI training (preferred)
