Your Opportunity:

We are currently recruiting for a full-time Triage Clinician (1.0 FTE) to join our Interprofessional Primary Care team.

The Triage and Float Clinician is responsible for assessing and prioritizing patient needs within the primary care centralized hub(s). This role ensures timely access to appropriate care by evaluating symptoms and functioning, assessing social determinants of health, determining urgency, and directing patients to the most suitable healthcare services. The clinician will collaborate closely with primary care providers, interdisciplinary healthcare providers and community resources to ensure seamless patient care.

A Successful Candidate in This Role Will Have:

  • Post secondary degree/education at the Bachelors or Master's level education in Nursing, Social Work or other healthcare related field.
  • Registration with the related college in good standing required.
  • Minimum of 5 years' experience in clinical practice, preferably in primary care, emergency care, or mental health setting.
  • Skills:
  • Strong clinical assessment and decision-making skills.
  • Ability to manage high volumes of patient interactions efficiently.
  • Proficiency in EMR systems and documentation.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced, collaborative environment.
  • Experience and training in crisis intervention and de-escalation techniques.
  • Familiarity and relationships with the GHHN community, health and social services preferred.
  • Experience in brief mental health and/or social care interventions.

Duties and Responsibilities:

· Patient Assessment & Triage: Conduct brief initial patient assessments via phone, virtual consultations, or in-person to determine the level of urgency and appropriate care pathway.

· Clinical Decision-Making: Utilize clinical judgment and evidence-based guidelines to triage patients to primary care providers, secondary care, tertiary care, community services, emergency services, interdisciplinary team-based care services or other specialized care and programs.

· Care Coordination: Work closely with HFHT physicians, NPs, PAs, nurses, mental health counselors, social workers, registered dietitians, pharmacists, physiotherapists and psychiatrists while also coordinating with our community and health care partners to facilitate timely referrals and follow-up care.

· Brief Intervention and Case Management: Provide brief, solution-focused interventions and carry a small patient caseload as patients are connected to the appropriate resources.

· Crisis Intervention: Identify and manage urgent cases, providing de-escalation support and appropriate crisis referrals.

· Patient Education: Provide patients and families with guidance on symptom management, self-care strategies, and when to seek further medical attention.

· Documentation & Compliance: Maintain accurate and timely documentation in electronic medical records (EMRs), adhering to regulatory and organizational standards.

· Quality Improvement: Participate in ongoing training, case reviews, track and manage data and process improvement initiatives to enhance triage efficiency and patient care outcomes.

· Float: May be required to carry a small patient case load or back-up mental health coverage if capacity allows.
