Position Overview
This position is responsible for the provision of clinical social work services to hospital patients, families and providing excellent patient care within an interdisciplinary team setting. The Social Worker will provide comprehensive assessments, treatment planning, personal and group education along with supportive counselling, resource identification, referrals and linking. The primary area of coverage for this role will be within the Bariatric Medicine clinic.
Education
- Must be a member of the Ontario College of Social Workers and Social Service Workers, or be a candidate for certification
- Master of Social Work from an accredited school of social work, or equivalent degree, as recognized by the Collegeof Registered Social Workers and Social Service Workers
Experience
- Minimum of 5 years of social work experience
- Demonstrated interest in adults with bariatric and metabolic conditions
- Knowledge of psychosocial issues related to weight management an asset
- Experience working with adults and mental health
- Excellent motivational interviewing skills required
- Creative and vibrant individual with strong skills in motivation and confidence building techniques and interventions
- Knowledge of and experience working with community partners/resources
- Ability to develop and maintain positive working relationships
- Program development, management and evaluation skills an asset
- Keen attention to service quality and innovation
- Demonstrated ability to provide comprehensive assessment and treatment planning
- Demonstrated ability to provide collaboration and consultation with inter- professional team and community-based service providers
- Ability to participate in education delivery to social work and other health professionals, students, patients and families, colleagues and community agencies
- Ability to participate in professional, program and hospital development, planning, implementation and evaluation of service
- Strong written and verbal interpersonal and communication skills required
- Demonstrated family centered philosophy of care provision
- Demonstrated ability to welcome change and manage it innovatively
- Good work and attendance record required
