Position Overview:

We are seeking a highly skilled and compassionate Advanced Practice Nurse (APN) to join our virtual care team. This role involves delivering high-quality healthcare services to patients through virtual platforms, ensuring access to care is convenient, efficient, and patient-centered. The ideal candidate will possess advanced clinical expertise, excellent communication skills, and a commitment to improving health outcomes through technology-enabled care.

Key Responsibilities:

  • Patient Care:
  • Conduct virtual consultations to assess, diagnose, and treat patients across a broad spectrum of health conditions.
  • Develop and implement individualized care plans based on patient needs.
  • Prescribe medications, order diagnostic tests, and interpret results as needed.
  • Provide health education and counseling to patients and their families.
  • Technology Use:
  • Utilize secure virtual care platforms to connect with patients and document encounters.
  • Ensure compliance with data privacy and security regulations.
  • Stay updated on telehealth technologies and recommend process improvements when applicable.
  • Care Coordination:
  • Collaborate with interdisciplinary teams, including physicians, specialists, and allied health professionals, to ensure continuity of care.
  • Refer patients to in-person services or specialists when necessary.
  • Monitor patient outcomes and follow up to assess treatment effectiveness.
  • Administrative Duties:
  • Maintain accurate and thorough medical records in electronic health systems.
  • Participate in quality assurance initiatives to enhance patient care delivery.
  • Stay informed of telehealth policies and regulatory requirements.

Qualifications:

  • Education: Master's or Doctoral degree in Nursing (MSN/DNP) from an accredited institution.
  • Licensure: Active and unrestricted Advanced Practice Nurse license (e.g., Nurse Practitioner or Clinical Nurse Specialist) in the jurisdiction of practice.
  • Certification: Board-certified in a relevant specialty (e.g., Family, Adult-Gerontology, or Pediatric Nurse Practitioner).
  • Experience: Minimum 3-5 years of clinical experience, with prior experience in telehealth or virtual care preferred.
  • Skills:
  • Strong clinical assessment and decision-making skills.
  • Proficiency in using electronic medical records (EMR) and telehealth platforms.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage a diverse patient population remotely.

Preferred Attributes:

  • Experience managing chronic illnesses and preventive care in a virtual setting.
  • Multilingual skills to serve a diverse patient population.
  • Knowledge of healthcare technologies, digital health tools, and telemedicine best practices.

Work Environment:

This is a remote position that requires a secure, private space for conducting virtual consultations. Candidates must have reliable high-speed internet access and the ability to work flexible hours, including evenings or weekends, to accommodate patient needs.
