Title: ACTIVATION Coordinator
Reports to: Executive Director
Employment Status: **Permanent Full-time
Job Summary: The Activation Coordinator will be responsible for the development and implementation of activation activities to help persons living with dementia remain active in their homes and in group settings. Through the use of games, sports, hobbies, crafts and more, they will support families in maintaining the physical and emotional well-being of their loved one living with a form of dementia. They will also be responsible for navigating clients in the community health promotion programs.
The Activation Coordinator will be a part of the client's circle of care and will support in:
- Develop and facilitate in-house activation programs with clients on a weekly basis to improve clients' wellbeing across several domains (physical, emotional, social, intellectual) as well as provide respite for the care partners.
- Carry out identified recreation programs as well as provide respite for the care partner.
- Provide education for families, staff and the general public regarding appropriate recreational activities for persons living with dementia.
- Marketing and growth of the activation program.
- Evaluate program at specified intervals.
Knowledge/Skills/Abilities
- Experience working with persons living with dementia and their families.
- Experience designing recreational programs to support individual needs as well as facilitating group activities.
- Knowledge of community health care agencies.
- Current Standard First Aid/CPR certification (or willingness to obtain).
- Ability to work cooperatively with volunteers, staff and community partners.
- Competency with Microsoft Office applications.
- Valid driver's license, regular access to a vehicle and verification that the vehicles is insured for work purposes.
- GPA/Montessori certification or willing to obtain
Responsibilities:
- Develop and maintain a caseload of clients.
- Effectively participate in care conferencing. Assessments of clients and determine eligibility and appropriateness to in-home Activation program and support an interdisciplinary team to implement the activation care plan.
- Conduct in-house programs with clients on a weekly basis in order to carry out identified recreation programs as well as provide respite for the care partner.
- Follow established job routines on a daily basis.
- Ensure that care provided is documented as required in a timely manner using the assigned documentation tools.
- Provide education to families and/or in-home care providers regarding appropriate activities for the client.
- Evaluate the effectiveness of the programming for the individual.
- Act as a resource for inquiries from the general public regarding recreation/activation.
- Market the in-home Activation program to health care providers, referring agencies and the general public
- Maintain program statistics and submit monthly.
- Assume responsibility of personal upgrading in knowledge of Alzheimer's disease, related dementias and on- going professional development.
- Participate in fundraising and other team activities.
- Assist with recruitment, training, and scheduling of activation volunteers
Other Duties
- Assume other functions as may be assigned by the Program Manager and/or Executive Director.
Job Qualifications
- Post-secondary degree or diploma in Recreation Therapy or Health Promotion. Other health disciplines may be considered given appropriate combination of education and experience.
Travel Requirements
- Valid Ontario Driver's License, Due to the need to transport materials, equipment and confidential client information (e.g., pre and post assessments, evaluation feedback, etc.), reliable and appropriate access to transportation is required
- Travel across society service area as required.
Physical Demands
- performance of general office duties.
- occasional longer periods of driving.
- bending and kneeling based on type of activity
- Transferring of program equipment and supplies requires the ability to lift up to 25 pounds
Working Conditions
- While not the norm, program participants may exhibit unpredictable behaviour that could include verbal and/or physical responses
