Job Summary
The incumbent will work half time in the Personal Care Home Program and half time in the Community Support Program, with a set schedule.
Personal Care Home Program: The Personal Care Home (PCH) Program serves older adults and individuals with complex medical, functional, and cognitive needs who require 24-hour supportive care. Residents commonly present with multiple chronic conditions (e.g., dementia, cardiovascular disease, diabetes, renal disease), frailty, swallowing difficulties, and an increased risk of malnutrition, dehydration, and unintentional weight loss.
The Clinical Dietitian II provides clinical nutrition services to PCH residents, including nutrition assessment, care planning, implementation, and ongoing monitoring to support nutritional status, functional ability, and quality of life. The role involves close collaboration with nursing, speech-language pathology, physicians, and food services to support therapeutic and texture-modified diets, enteral nutrition, and palliative nutrition care, while promoting resident dignity, choice, and person-centred care.
The Clinical Dietitian II also functions as member of the interdisciplinary team responsible for the monitoring and licensing of Personal Care Homes. This role includes reviewing nutrition- and food service–related activities and reports for assigned PCHs to ensure compliance with the Personal Care Home Program Operational Standards (revised April 2007) and the Personal Care Home Regulations Monitoring activities may occasionally require evening or weekend work. The dietitian also provides nutrition counselling and education to residents and PCH staff; participates in interdisciplinary meetings, annual reviews, complaint investigations, and inspections; and completes required monthly statistical reporting.
Community Support Program:
The Community Supports Program serves individuals living in the community who require nutrition intervention to maintain health, manage chronic disease, and prevent functional decline. This population includes older adults, adults with chronic conditions, individuals with disabilities, and those experiencing social, economic, or food security challenges.
The Clinical Dietitian II with the Community Supports Program is a key member of the interdisciplinary team. Responsibilities include the assessment, planning, implementation, and evaluation of nutrition services, as well as responding to referrals from health professionals for community-based nutrition assessment and counselling. The Dietitian provides nutrition education and consultative support to Community Supports staff and clients, and contributes nutrition expertise through participation in internal and external committees, resource development, and health promotion initiatives.
Nutrition care focuses on individualized assessment, education, and counselling to support chronic disease management, optimize nutritional intake, and promote independence. The dietitian provides medical nutrition therapy, supports malnutrition prevention and recovery, and addresses barriers such as access to food, cooking skills, and health literacy. Collaboration with home care and community agencies is essential to supporting safe transitions, aging in place, and improved health outcomes.
In both programs, the dietitian will support program quality improvement initiatives; participate in evidence informed practice, education and community capacity building; assist with the overall coordination of nutrition services and mentor other dietitians and team members as appropriate. Duties of this position may change based on program operational requirements. The dietitian supports the NL Health Services Dietetic Internship Program through contribution of clinical expertise and/or preceptorship of dietetic interns and offers clinical education experiences to other students as required.
This position reports to the Manager of Personal Care Home Program and Division manager of Community Supports Program and maintains accountability to the Provincial Professional Practice Consultant for Dietitians for integrity and professional practice.
