The Licensed Practical Nurse Care Manager (LPN-CM) is the primary onsite licensed leader responsible for coordinating and supporting the delivery of resident care in accordance with the ANBLPN Standards of Practice, Code of Ethics, and New Brunswick Social Development Guidelines. The LPN-CM provides clinical oversight within the LPN scope, supervises unregulated care providers, ensures safe and evidence-informed practice, maintains accurate documentation, and promotes respectful, resident-centered care.

The role requires strong leadership, sound clinical judgment, and effective communication with residents, families, interdisciplinary partners, and the management team.

Do you posses:

· Active registration with the Association of New Brunswick Licensed Practical Nurses (ANBLPN)

· Successful completion of a recognized Practical Nursing Program

· 3–5 years of LPN experience in care coordination or long-term care

· Leadership or supervisory experience preferred

· Experience with Alzheimer's disease and other dementias

· Back in Form or equivalent safe-handling certification

· Current First Aid and CPR certification

· Strong leadership, communication, and critical-thinking skills

· Ability to function independently as the sole onsite licensed professional

· Ability to work effectively in a fast-paced environment

· Proficiency in Microsoft Office and electronic documentation systems

· Valid driver's license

Job Responsibilities:

  • The LPN-CM provides clinical leadership and coordination while practicing within the LPN scope as defined by ANBLPN. Responsibilities include directing care delivery, supporting staff, facilitating communication, and ensuring regulatory, professional, and organizational compliance.
