Flexible Assertive Community Treatment is a new service delivery model in community mental health. This new FACTT DD team will provide intensive community based treatment and support to individuals with a Dual Diagnosis (Intellectual Disability and Mental Illness). The team will work closely with individuals, their caregivers and family members to ensure they are well supported and have access to appropriate resources and treatment in the community. The Social Worker on FACTT DD is part of an interdisciplinary treatment team. The team is located off-site and provides service to clients in the community.
Duties:
- Use best-practice and innovative approaches to provide client-centered services including assessment, treatment and psycho-social rehabilitation.
- Regularly update client service plans (including crisis plans).
- Provide direct assistance and support to clients regarding housing, employment, relationships & leisure activities, activities of daily living, and transportation.
- Work collaboratively, cooperatively and effectively within the FACTT DD team and with other community-based agencies including hospital inpatient units and emergency.
- Demonstrate thorough knowledge of the theories and practices of a professional social worker as they relate to family, individual and systems interventions.
- Provide support to family and caregivers of FACTT DD clients as needed.
- Provide crisis assessment and intervention to clients on a 24 hour basis (rotating on-call evenings and weekends).
- Participate in daily FACTT DD team meetings to review clients and organize daily assignments.
- Complete and maintain all necessary FACTT DD team documentation (CRMS). Documentation to be maintained in accordance with hospital and college policies and timelines.
- Advocate with community partners for services and opportunities for ACTT clients.
- Be familiar with the ROHCG Strategic Plan and the Goals and Objectives of the Community Mental Health Program.
- Keep current with hospital policies and procedures and applicable legislation (Mental Health Act etc.)
- Complete and maintain all requirements for registration with OCSWSSW.
- Keep up to date regarding professional standards by reviewing current literature, attending education events, networking with colleagues, etc.
- Regularly input Workload Measurement.
- Facilitate lectures and information sessions to clients and the general public.
- Participate actively in relevant Program, service, hospital and community committees including academic (education and research), advocacy and system development as required.
- Participate actively in hospital monthly Social Work meetings, Peer Consultation groups, committees, and quality assurance requirements.
- .Strong comfort level working in an automated environment that uses an Electronic Health Record system.
- Supervise Social Work students.
- Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of The Royal.
- Ensure a work environment that is conducive to The Royal's Anti-Racism, Discrimination & Harassment-Free Workplace Policy.
- Perform other duties as assigned.
Qualifications:
- Master's degree in Social Work.
- Current registration and member in good standing with the Ontario College of Social Workers & Social Service Workers.
- Minimum of three (3) years of clinical Social Work experience working with Dually Diagnosed clients in the mental health and/ or a developmental sector setting is required.
- Willingness to work variable hours, evenings and weekends.
- Knowledge of MHA.
- Familiarity with community resources for the Dual Diagnosis population including DSOER process
- Collaborative approach to working with community partners.
- Strong conflict resolution abilities.
- Excellent advocacy skills.
- Proven presentation, training and group facilitation skills.
- Demonstrated ability to work independently.
- Effective team skills.
- Able to work in a progressively more automated environment.
- Requirement to work regularly scheduled day, evening and weekend shifts and to be available for on-call duty.
- Valid Driver's license and access to a reliable vehicle.
- Bilingual (French/English) is considered an asset. English level A- is mandatory in oral expression/comprehension, reading comprehension and written expression.
