ONA /hr $63.18/hr

RESPONSIBILITIES:

• Provides expert nursing care and co-ordinate complex care challenges across a variety of settings through the roles of expert clinician, educator, researcher, consultant and change agent to promote quality outcomes for patients with heart failure, and minor strokes in alignment with hospital priorities
• Performs advanced health assessment and clinical decision-making within a clinical specialty, emphasizing health promotion, disease prevention and collaborative management strategies to positively affect health outcomes
• Participates in the development, monitoring and evaluation of protocols/guidelines/standards of care/programs for the heart failure, minor stroke patient population by offering rationale/support based on evidence and clinical expertise
• Participates in quality improvement or research designed to evaluate patient care outcomes and improve quality for care
• Provides consultation to nursing staff, managers, physicians and other team members
• Participates and provides leadership on interprofessional committees related to development of policies, procedures, protocols, medical directives, guidelines, clinical pathways, standards, education or research based on evidence based best procedures and guidelines
• Demonstrate QCH values: collaboration, accountability, innovation, respect

• Master's degree in nursing (other relevant master's programs may be considered)
• Registered with the College of Nurses of Ontario (CNO)
• Minimum of 5 years recent relevant clinical nursing experience
• Minimum of 3 years recent experience with the chronic disease population, preferably experience managing heart failure
• Excellent organizational, interpersonal, analytical, problem solving and decision-making skills
• Demonstrated teaching and consultative skills
• Expert Communicator
• Previous leadership experience
• Previous adult education/program development experience
• Demonstrated ability to meet deadlines/timelines and respond to shifting priorities
• Demonstrated commitment to own continuing academic growth and development

• Member of the Registered Nurses Association of Ontario (RNAO)
• Member of the Canadian Nurses Association (CNA)
• Canadian Council of Cardiovascular Nurse CNA Certification
• Experience with clinical documentation in an Electronic Health Record preferably EPIC and eClinical Viewer
