Who we are looking for:

  • Must have a valid BCCNM license
  • 1 year experience in Acute Community Care
  • Experience with 1:1 lifts
  • Experience with Tube feeding
  • Experience with Suction and Nebulizer
  • Valid Driver's license
  • Valid CPR and TB Test
  • Immunization records including COVID-19
  • A clear Police Criminal Record with Vulnerable Sector Screening Check
  • Ability to communicate effectively, both verbally and in writing
  • Physical ability to carry out the duties of the position
  • Ability to work independently and in cooperation with others
  • Ability to organize and prioritize
  • Ability to establish and maintain a trusting relationship with clients
  • Ability to observe and recognize changes in clients, and communicate them clearly with the case manager

Responsibilities include (not limited to)

  • Provide a supportive environment for the client; contributing to the development an modification of the clients care plan
  • Execute on the clients care plan as outlined and reviewed by the Case Manager
  • Attend and provide input into the interdisciplinary care planning meetings
  • Plan, organize and establish client care plan priorities when providing care
  • Respond to unexpected events and changing client needs within the scope of practice and competency guidelines
  • Perform nursing care according to the individual care plan, evaluate outcomes, record observations and report problems and/or changes to case manager
  • Document and update relevant care and procedure information to ensure client safety
  • Assists clients with activities of daily living such as feeding, patient transfers, bathing, dressing, skin care, wound care, toileting needs.
  • Medication administration according to the care plan
  • Perform within the limits of your experience and knowledge, while practicing safely and competently within the scope of practice
  • Assist in the orientation and education of new staff
