About the Family Health Team

Village Family Health Team Name is an interprofessional primary care organization committed to providing comprehensive, patient-centred care to our community. Our team includes family physicians, nurse practitioners, nurses, social workers, dietitians, and allied health professionals who work collaboratively to support patients across the lifespan.

Position Summary

We are seeking a Chiropodist to join our Family Health Team and provide high-quality foot care services to patients with acute, chronic, and preventative foot health needs. The successful candidate will work collaboratively within an interdisciplinary team to support improved mobility, function, and overall health outcomes, particularly for patients with chronic disease such as diabetes.

Qualifications:

  • Registered Chiropodist in good standing with the College of Chiropodist of Ontario with ability to work full scope including administering substance by injection and prescribing drugs

  • Graduate of an accredited college of podiatric/ chiropodial medicine

  • Current in evidence based best practices and clinical practices

  • Training in Infection Prevention and Control is an asset

  • Work effectively, both independently and as part of an interprofessional team

  • Flexibility and ability to adapt to change

  • Ability to use electronic documentation for health records. Village FHT uses Accuro.

-Experience working with diverse and/or vulnerable populations is an asset

Key Responsibilities

  • Provide assessment, diagnosis, and treatment of foot and lower limb conditions within the scope of chiropody practice
  • Deliver preventative foot care, wound care, nail and skin care, and biomechanical assessments
  • Manage patients with chronic conditions (e.g., diabetes, vascular disease, arthritis)
  • Prescribe and dispense orthotics and footwear as appropriate
  • Maintain accurate and timely clinical documentation in the electronic medical record (EMR)
  • Collaborate with physicians, nurse practitioners, and other allied health professionals
  • Provide patient education on foot health, self-care, and injury prevention
  • Participate in quality improvement initiatives and team meetings as required

· Provide home visits to partner health organizations as required

· - Provide quarterly reporting as required

Hours of Work: 8-10 hours/week

Status: Contract 1 day/week
