SUMMARY OF POSITION

The Nursing Practice Facilitator is a key member of the Assertive Community Treatment (ACT) Team, providing leadership, coordination, and advanced nursing expertise to support the delivery of community-based, recovery-oriented mental health services. The Nursing Practice Facilitator ensures that nursing care within the ACT model aligns with evidence-based practices, regulatory standards, and the holistic needs of clients experiencing severe and persistent mental illness, often with co-occurring substance use disorders and/or homelessness. Key responsibilities will include clinical leadership and education both within the ACT Team as well as across the agency, client care and coordination, team collaboration, mentorship, program development and evaluation.

The Nursing Practice Facilitator will demonstrate the principles of client-centered care by practicing comprehensive bio-psychosocial assessments, mental status and physical health assessments; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing intensive treatment, rehabilitation, and community support services.

HOURS OF WORK

The Nursing Practice Facilitator position will work a thirty-five (35) hour week, the hours being flexible to the needs of the program, with one afternoon shift (12pm-8pm) per week. This role participates in the agency's on-call rotation.

ESSENTIAL QUALIFICATIONS

  • Current registration as a Registered Nurse (RN) with the College of Nurses of Ontario
  • Master's degree in Nursing, education, or related field considered a strong asset
  • Preference given to candidates certified in, or in the process of completing courses toward the certificate in Canadian Psychiatric and Mental Health Nursing
  • Minimum 3-5 years experience in a community mental health setting working with adults recovering from serious mental health illness, preferably in community psychiatry or ACTT/Intensive Case Management settings
  • Experience providing leadership in a multidisciplinary environment
  • Knowledge of mental health resources within Waterloo Region considered an asset
  • Certification in First Aid/CPR and suicide risk assessment preferred
  • Demonstrated skill in applying the principles of Psycho-Social Rehabilitation, Recovery and case management
  • Working knowledge of the Mental Health Act and other relevant legislation
  • Willing to travel extensively within the catchment area
  • Must have a valid Class G driver's license, relevant insurance and use of a reliable vehicle
  • Proficient in PC skills (MS Office Suite)
  • Provide a satisfactory police vulnerable sector check
