Indigenous Patient Navigator

Job Summary:
Under the supervision of the ATC Health Director, and in collaboration with the Indigenous Hospital Liaison at the Northern Lights Regional Health Centre (NLRHC) and Willow Square Long Term Care Facility, the Indigenous Patient Navigator position is responsible for developing connections with Indigenous clients entering, leaving, and continuing care. The position supports clients by advocating on their behalf, helping them navigate the health care system, and providing bridging supports during the transition from hospital to home.
Duties and Responsibilities:
- Act as liaison for all Indigenous patients/families and NLRHC-AHS staff.
- Collaborate with the social worker and discharge coordinator to ensure all needs are considered for the Indigenous patient and their family.
- Advocate with Indigenous patients by interpreting cultural and language differences and providing direct guidance.
- Support Indigenous patients throughout their health care journey, ensuring cultural practices and values are incorporated into their health care plan.
- Coordinate discharge planning, including return appointments, follow-up, and links to community care.
- Support health education for Indigenous patients in conjunction with the Health Care Team by identifying problems, concerns and unmet needs.
- Work with the multi-disciplinary teams to address Indigenous patients concerns, resolve issues, and identify discharge barriers.
- Advise Indigenous patients of all services available to them under Non-Insured Health Benefits (NIHB).
- Perform other duties and responsibilities as assigned to support organizational objectives.
Qualifications and Skills:
- Minimum High School Diploma or G.E.D. equivalency is required.
- 3-5 years direct experience working within a similar role.
- An equivalent combination of training and experience may be considered.
- Knowledge of local Indigenous cultures, history, ceremonies, and traditions will be considered an asset.
- Excellent written and verbal communication skills, including knowledge of basic medical terminology.
- Ability to build and maintain trust and respect with Indigenous clients and AHS staff.
- Excellent time management skills and ability to multi-task and prioritize work.
- Ability to work in a confidential, fast-paced environment with minimal supervision while being self-directed.
- Ability to speak Dene or Cree will be considered an asset.
Requirements:
- Clear Criminal Record Check with Vulnerable Sector.
- Clear Child Intervention Record Check.
- Class 5 License with a clear Drivers Abstract.
- Ability to work occasional evenings and weekends.
Work Conditions:
- Hospital and Continuing Care settings.
- Must be flexible based on client circumstances.
