POSITION SUMMARY
Under the direction of the Manager, Community Services, the Community Health Promoter is an energetic, self-starter who has a baccalaureate Degree in a health-related discipline (Nursing, Dietitian, Occupational Therapy, Physical Therapy, Social Work, Speech Language Pathology or other). The Community Health Promoter (health promoter) will focus on diabetes prevention and Smoke Free Ontario programs and is responsible for strengthening the capacity of Indigenous individuals, families and communities to manage their health and well-being. This position will work with community organizations to develop and deliver culturally appropriately strategies and activities that will promote and engage a Smoke Free Ontario and diabetes prevention plan to Indigenous residents and clients of Anishnawbe Mushkiki in Thunder Bay.
DUTIES AND RESPONSIBILITIES HIGHLIGHTS
· Responsible for the development and coordination of the overall programming objectives and work plans for designated community health programs.
· Develops overall annual plan for designated health programs.
· Responsible to promote diabetes prevention programs, smoking cessation and responsible traditional tobacco use and other chronic disease prevention and education to Indigenous people and clients of Anishnawbe Mushkiki in Thunder Bay; in accordance with Ministry of funding guidelines, requirements and programming priority indicators.
· Facilitates weekly support and education groups for community members that are at risk for, or are managing diabetes, as needed.
· Facilitates the successful implementation of the Centre of Addiction and Mental Health's (CAMH) STOP Program through the Nicotine Replacement Therapy (NRT) at the Anishnawbe Mushkiki Community Health & Wellness Clinic through education and promotion to clinic staff.
· Executes the NRT with clients.
· Prepares all materials required for workshops, including pre and post requirements.
· Screens clients for diabetes and those at risk through referrals to partner organizations and providers.
· Plans, coordinates and executes activities, through collaboration with in-house dietitian to deliver programming that promotes diabetes prevention awareness with the end goal of Healthy Eating and Active Living. Assists patients with modifiable risk factors.
· Contacts new client applicants of Anishnawbe Mushkiki for intake scheduling. Completes intake information appointments with new clients.
· Documents, as required, in the electronic medical record system.
· Coordinates and finalizes overall reporting for presentation to the Manager, Community Services, prior to the presentation to the funder.
QUALIFICATIONS
· Degree in health-related discipline (Nursing, Dietitian, OT, PT, Social Work, SLP or other) or in public health or related health care education or equivalency
· Minimum three years' related work experience
· Experience with smoking cessation programs, diabetes prevention and treatment, is considered an asset
· Use of an automobile, valid driver's license and appropriate insurance are required
· Tuberculin (TB) Skin Test, satisfactory Criminal Record Check including Vulnerable Sector Screening Check
· Current CPR/First Aid certification is required throughout employment
· Good computer skills and knowledge
· Superior organizational and time management skills
· Effective communication skills- verbal, written and electronic format
· Demonstrated ability to work effectively as a team member in a multi-disciplinary environment
· Proactive and able to work independently, with minimal supervision
· Sound decision making and problem-solving skills
· Knowledge and experience working with the Indigenous Community
· Knowledge and awareness of Indigenous culture and traditions is an asset
· Ability to speak the Ojibway and/or Cree languages is an asset
