Clinic Nurse

The Clinic Nurse provides direct clinical care to patients while supporting them in navigating the healthcare system. This role combines hands-on nursing practice with care coordination, patient education, and communication across the care team. The nurse ensures clinical information is accurately documented, follow-up steps are completed, and patients receive timely support to access the right resources.
Serefin Health Clinic founded in 2010, is Canada's first health management organization. We work to address systemic gaps in the healthcare system by creating coordinated, patient-centered solutions through medical, nursing, and technical collaboration.
This is an onsite role at our Toronto clinic, Monday to Friday.
Scope of Role
Clinical Care
- Assess patients of all ages in a clinic setting.
- Perform examinations, collect samples, conduct tests, and administer vaccines.
- Provide direct patient care, support, and health education.
- Triage patient concerns using standardized tools and manage urgent situations within scope.
- Perform phlebotomy or be willing to learn.
- Complete accurate electronic charting (EC) in a timely manner.
Patient Communication
- Respond to patient calls and emails to triage concerns and provide guidance.
- Offer advice on condition management, self-care strategies, and available resources.
- Support appointment preparation, follow-up instructions, and care-plan adherence.
Care Coordination
- Transfer clinical information into the care-coordination platform.
- Track patient progress, system interactions, and follow-up needs.
- Communicate with families and healthcare providers to ensure clarity and continuity.
- Facilitate access to community programs and support services.
Team & Clinic Operations
- Collaborate with an interprofessional team to deliver high-quality care.
- Participate in quality improvement initiatives and help refine clinic processes.
- Contribute to practice growth by supporting workflow improvements and patient experience optimization.
Nice to Have – Medical Device Reprocessing (MDR)
- Possesses the Medical Device Reprocessing Techniques certificate
- Clean, disinfect, sterilize, and maintain reusable medical/surgical instruments as needed for the clinic
Required Experience
Education & Credentials
- Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
- Current BLS certification; additional patient assessment training is an asset.
Clinical Background
- 2–5 years of acute care experience (e.g., Emergency, ICU, Mental Health/Psychiatry, Behavioural Health).
- Experience supporting elderly individuals and patients with complex medical, mental health, or addiction-related needs.
- Ability to triage patient situations independently using standardized assessment tools.
Skills & Attributes
- Strong clinical assessment, critical-thinking, and decision-making abilities.
- Excellent communication and interpersonal skills, both written and verbal.
- Skilled in patient teaching and supporting behaviour change.
- Comfortable navigating challenging conversations with empathy and professionalism.
- Able to manage multiple priorities in a fast-paced environment.
- Proficient with technology, clinic software, and phone systems.
- Demonstrated commitment to ongoing learning and professional development.
- Satisfactory attendance record and up-to-date Police Reference Check.
- Proficiency in English; French or other languages considered an asset.
