About the Role
As a Community Inclusion Worker, you will assist a person who requests support to participate in personal, social, vocational, and volunteer opportunities within the community. The services are determined by the person and based on their specific goals and preferences. You'll encourage and empower the person to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Your natural ability to care for others and work collaboratively towards a goal will allow you to influence your person's success.
Schedule- 30 hours/week
Core Responsibilities
Ensure the supported person's rights and choices are respected and advocated as neededInteract effectively with people we supportEmpower the person we support to be full citizens of their communityCoordinate, implement, and facilitate the person's participation in community-based social, vocational and volunteer activitiesFoster positive relationships with person's family members, professionals, co-workers, and members of the communityParticipate in the development, implementation, and documentation of the Individual Service PlanRecord the daily activity and personal health information (PHI) of the person we support on our case management databaseExcellent understanding of behaviour management principles and strategies including mental health needsYou will have
Grade 12 educationCommunity Support Worker Diploma or equivalentMinimum 2 years of experience in community social services/health careBasic American Sign Language proficiencyCurrent Emergency First Aid/CPR and Food Safe certificatesBehavior management trainingAbility to effectively communicate (verbal & written) with team members, professionals, families, and community membersNon-verbal communication skills, knowledge supporting deaf/blind is an assetValid BC Driver's LicenseSatisfactory Driver's Abstract & reliable vehicle with $2,000,000 third-party liability insuranceCriminal record clearance for children and vulnerable adultsModerate computer literacyEligibility to work in CanadaWhy join Pacific Coast Community Resources?
