About Us

The Lower Columbia Community Health Centre Network Society is a not-for-profit organization committed to providing holistic, accessible healthcare to all – especially the vulnerable populations we serve. Located amidst the stunning natural beauty of the Kootenay region, our organization owns and operates the Lower Columbia CHC, a new community health centre in Trail, BC. We are looking to add to our multidisciplinary team-based care that delivers comprehensive care addressing the physical, mental, and social determinants of health.

Your Role

LPNs are a skilled healthcare worker who can contribute a large amount to the delivery of clinical care in a Primary Care setting. LPNs work in partnership with FPs, NPs, RNs, and other members of the health care team to provide primary care nursing services primarily to patients with stable and predictable health needs. LPNs can work autonomously to support more complex care with additional education, training, and/or supervision.

Hours of Operation:

Monday-Friday 8-5, Wednesday 7-7, Saturday 9-1. Employees may be required to work the evening or weekend shifts depending on operational need and team coverage.

Key Responsibilities

  • LPNs provide preliminary patient assessment that facilitate patient care by an FP or NP
  • Understand the LPN scope of practice and perform the following duties within that scope and with the required education and competence, seeking supervision when required:
  • Ear syringing
  • Vaccination administration
  • Allergy desensitization shots
  • Taking, recording vital signs and reporting changes in patient condition
  • Weighing babies/kids
  • Taking and processing urine samples
  • Dressing changes
  • Rooming patients and preparing them for their visit based on type of appointment
  • Screening/assessment to inform the development of the health care plan, including pre-visit checklists and questionnaires
  • Conducting individual and established group education to address health topics and support chronic disease prevention and self-management (e.g., smoking cessation, problematic alcohol and substance use, physical activity, and healthy eating)
  • Provision of therapeutic interventions in accordance with the health care plan and in collaboration with the interdisciplinary team, including algorithm-based panel management and cancer screening offers of care (HPV self-swabs, FIT tests, mammography, PSA testing)
  • Monitoring/evaluation of the health care plan
  • Perform assigned tasks such as screening for metabolic conditions and monitoring stable disease (hypertension, hypercholesterolemia, type 2 diabetes, hypothyroidism, osteoporosis)
  • Perform tasks opportunistically at the time a patient presents for an appointment or proactively through outreach with phone, text, email (with patient consent), or mail.

Qualifications and Skills

  • Successful completion of a recognized practical nursing program
  • Current LPN registration and license with the BC College of Nurses and Midwives
  • Immunization certification would be considered an asset.
  • Previous clinical experience in a primary care setting would be considered an asset.
  • Valid Class 5 Driver's Licence

  • Ability to provide basic patient care, including hygiene, mobility, and comfort
  • Familiarity with and ability to operate various medical equipment
  • Accurate and thorough documentation of patient care and observations using electronic health records (EMR) and other computer-based systems.
  • Ability to organize workload and prioritize tasks effectively
  • Being able to adjust to different work environments and schedules
  • Ability to stand and move for extended periods of time
  • Understand the ethical principles and their application in nursing practice
  • Demonstrates honesty, integrity, and ethical judgment.
  • Knowledge of relevant laws and regulations governing nursing practice
  • Taking ownership of one's actions and decisions and being accountable for the quality and safety of patient care.
