The Case Manager is responsible for managing a caseload of patients; creating a milieu for empowerment in the provision of patient focused care; promoting collaborative practice, coordination of care and continuity of patient care among team members; and integrates the components of evidence based practice to improve the quality of care provided to patients.
1. Graduate of an approved School of Nursing and currently registered with the CNO; B.Sc.N preferred
2. Demonstrated experience in case management
3. Demonstrated skills in program development
4. Demonstrated motivation to evaluate quality and cost
5. Demonstrated strong assessment skills for patients and families with complex needs
6. Demonstrated ability to coordinate the care of patients with complex needs with other team members (including family) and community agencies
7. Demonstrated ability to work effectively with a variety of health care professionals in the community
8. Demonstrated commitment to providing care that is based on the best practice as evidenced in the literature
9. Demonstrated leadership and decision- making abilities and be able to take acceptable risks
10. Demonstrated ability to adjust quickly to changing conditions - both patient conditions and environmental conditions
11. Demonstrated responsive to challenges and work well under pressure
12. Demonstrated exceptional communication and interpersonal skills
13. Demonstrated initiative and a self-confident individual
14. Demonstrated commitment to teaching and life-long learning
15. Demonstrated commitment to teaching and preceptorship of nursing students helping them to see the nursing leadership role
16. Ability to attend work regularly; demonstrated by an acceptable attendance record
