- Ability to incorporate knowledge of diversity, cultural safety and the determinants of health in the assessment, diagnosis and therapeutic management of residents and the evaluation of outcomes
- Integrating Indigenous traditional health/medicine practices into plan of care in collaboration with Indigenous elders and leaders of the field
- Ability to exercise independent judgement and to assess situations and problems efficiently and effectively
- Leadership skills, working well within an inter-professional team and ability to foster effective working relationships
- To provide clinical supervision, education and mentoring for nursing students, medical students and other learners including residents/families
- Develop and implement evaluation processes and identify strategies to improve clinical outcomes and manage risks
- Strong and effective verbal and communication skills
What You'll Do
- Deliver primary healthcare to residents in the ALC/CCC beds in the management of acute and chronic medical conditions, therapeutic management, health promotion and disease/injury to deliver comprehensive health services
- Conducts focused on health assessment using and adapting assessment tools and techniques based on resident needs
- Complete health history into resident's situations, including physical, psychosocial, emotional, cultural and ethnic dimensions of health
- Performs physical examination and identifies and interprets normal and abnormal findings
- Develops and implements individual care plans, liaison with physicians and other health care professionals and provide education to patients and families
- Provide clinical expertise in geriatric and palliative and continuity for LTC patients
- Mentor, coach and support excellence in care for patients/families, LTC and remote RN
- Successful completion of a Registered Nursing Program with certification as a Primary Health Care Nurse Practitioner.
- Registered with CNO as a Registered Nurse Extended Class (RN EC)
- Geriatric Certification from CAN or equivalent experience
- GPA and PIECES required
- BLS and ACLS (or willingness to complete)
- Commitment to Indigenous values and principles
- Willing and able to travel as required
