The Don Mills Family Health Team (DMFHT) provides primary health care, including health promotion and disease prevention and chronic disease management to the population of DMFHT catchment's area. The FHT comprises an interdisciplinary team of health care professionals who work in a collaborative model of care.

Job Description

  • Assess and Support Patients: Assist patients with overcoming physical, cognitive, and emotional barriers to maximize their independence in daily activities.
  • Collaborative Care Plans: Develop individualized care plans with direct input from clients and their families and to provide input and direction on the frequency and duration of needed OT interventions to meet client goals.
  • Interdisciplinary Collaboration: Work closely with physicians, nurses, social workers, and other professionals to ensure the best outcomes for clients.
  • Adaptive Equipment & Safety: Assess and train clients and families on the use of adaptive/assistive equipment, safety equipment, mobility aids, and environmental modifications.
  • Ongoing Monitoring: Reassess patients' progress and adjust care plans to ensure optimal outcomes.
  • Documentation: Maintain accurate documentation, including assessments, treatment plans, progress notes, and discharge summaries.

Required Qualifications:

  • Degree in Occupational Therapy from a recognized school of Occupational Therapy.
  • Member in good standing with the College of Occupational Therapists of Ontario.
  • Experience within the professional discipline is an asset.
  • Specialized courses to support areas of clinical expertise.
  • Ability to work independently.
  • Familiar with the routine use of valid framework and reliable outcome measures to support clinical interventions.
  • Demonstrated commitment to client and family centered care.
  • Demonstrated ability to recognize safety concerns, communicate effectively using a computer and/or mobile device.

Job Type: Part-time

Pay: $32.00-$35.00 per hour

Expected hours: 22.5 per week
