We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to serve as our Clinical Care Coordinator in the Chilliwack office. This position plays a key role in ensuring high-quality client care, supporting caregivers, and maintaining clinical excellence within our operations.
What You'll Do
- Conduct and document comprehensive assessments of clients' physical, psychosocial, and environmental needs.
- Finalizing service contracts with clients and families.
- Develop and update Plans of Care using nursing judgment and Comfort Keepers protocols.
- Monitor and evaluate client status regularly, adjusting care plans as needed.
- Perform skilled nursing interventions as required (e.g., first aid, medication management).
- Communicate effectively with caregivers, families, and healthcare professionals.
- Support caregiver orientation and skills development in coordination with HR/Admin.
- Participate in quality improvement activities and team meetings.
- Serve as on-call support when required and assist with emergency coordination.
- Promote a safe, professional, and compassionate care environment for both clients and staff.
- Support overall growth and success of the business through quality service and community engagement.

What We're Looking For
- Current registration as a Licensed Practical Nurse (LPN) with the BC College of Nurses and Midwives (BCCNM).
- Graduate of an accredited LPN program.
- Current Standard First Aid and CPR Certification.
- Evidence of up-to-date immunizations (or acceptable release).
- Vulnerable Sector Police Check
- Demonstrates proficiency in computer skills (Microsoft Office, scheduling systems, and documentation software).
- Strong interpersonal and communication skills.
- Ability to work independently and manage multiple priorities.
- Commitment to continuous learning and professional growth.
- Valid driver's licence and reliable vehicle
