Licensed Practical Nurse

Role Overview:

This position is a mobile Licensed Practical Nurse role requiring frequent travel between sites ( up to 50–75% of the time) to provide direct patient care. The successful candidate will administer OAT & other medications, witness ingestions, collaborate with interdisciplinary teams, and support patients through discharge planning. Accuracy in documentation and familiarity with medical coding systems are essential to ensure quality care and proper billing.

Duties:

  • Provide direct patient care, including administering medications
  • Collaborate with other healthcare professionals to develop and implement individualized care plans
  • Responsible for witnessing ingestions and dosing medications in a care facility
  • Participate in discharge planning activities, ensuring patients have appropriate follow-up care arrangements
  • Familiarity with medical coding systems for accurate documentation and billing purposes

Schedule:

  • , Monday-Friday, Alternating Weekends

Work Environment:

  • This role involves frequent travel between multiple care sites, with driving comprising a significant portion of the workday
  • Patient care is provided in various settings including clients homes & group homes & community
  • The position may involve exposure to patients with complex medical and behavioral health needs

Qualifications:

  • Must be a licensed practical nurse (LPN) in Alberta
  • Must have at least two (2) years of direct experience
  • Proven experience providing patient care in a mental health capacity is preferred
  • Previous experience working with OAT medications and the use of Narcan is preferred
  • Proven experience working in Home Care environment
  • Experience working independently is preferred
  • Previous experience in a healthcare setting preferred
  • Familiarity with health coaching principles and strategies

Pay: $27.00-$30.00 per hour
