Duties

• Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
• Monitor patients' health by taking vital signs such as blood pressure, pulse, temperature, and respiration; report any concerns to healthcare professionals promptly.
• Support medication administration following prescribed care plans and HIPAA regulations to ensure privacy and safety.
• Provide companionship and emotional support to promote mental well-being and reduce feelings of loneliness or isolation.
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas, laundry, and organizing supplies.
• Use specialized equipment like Hoyer lifts or resident lifts to safely transfer individuals with mobility challenges.
• Observe and document patient conditions meticulously, noting changes in behavior or health status for ongoing care planning.
• Follow established protocols for Alzheimer's care, dementia care, memory care, hospice care, or working with individuals with developmental disabilities or autism spectrum disorder.
• Collaborate with healthcare teams to develop and implement personalized care plans that meet each resident's unique needs.
• Ensure compliance with all safety standards including CPR certification, first aid procedures, and HIPAA privacy rules during daily operations.
• Engage actively in training sessions on patient observation techniques, medication safety, and emergency response procedures.

Qualifications

• Previous experience working in assisted living facilities, nursing homes, home health services, or group homes is highly preferred.
• Strong knowledge of patient observation techniques, vital signs monitoring, medication administration, and care plan implementation.
• Certification in CPR and First Aid is required; additional certifications such as HIPAA training or hospice care are advantageous.
• Experience working with people with disabilities—including developmental disabilities or autism—is a plus.
• Ability to lift heavy objects safely using proper techniques; physical stamina for long shifts involving standing or walking is essential.
• Excellent communication skills to effectively interact with residents, families, healthcare professionals, and team members.
• Compassionate attitude combined with patience and resilience when caring for individuals with complex needs such as dementia or Alzheimer's disease.
• Knowledge of infection control practices and maintaining confidentiality according to HIPAA regulations.
• Flexibility to work various shifts including evenings, weekends or holidays as needed.
