Runnymede LTC Home is a 200-bed long-term care home in Deseronto, Ontario dedicated to enriching the lives of our residents in partnership with UniversalCare Canada Inc., our family members, valued employees and community partners.
Our team is currently hiring for a permanent full-time RAI Coordinator, which is an existing vacancy. Reporting to the Director of Care, the RAI Coordinator position is a critical role with overall accountability for the planning, directing and coordinating of activities that support the efficiencies of the nursing care team and the safe and effective delivery of care to our residents.

As part of our team, your role as a RAI Coordinator contributes to our resident experiences in the following ways:
· Promotes a resident-centered care approach that engages residents, family members and employees to provide care that meets the individual needs of each resident.
· Supports with the implementation and maintenance of quality risk management activities to reduce and control potential or actual risks to the safety, security and health of all individuals, and to the safety and security of the long-term care home and conducts audits, as required.
· Promotes and ensures the provision of resident focused care and provides cognitive, emotional, spiritual, cultural, physical and social support to each resident.
· Participates as a member for assigned committees, as required.
· Assists with meal service, as required, ensuring the individual nutritional needs of all residents are met.
· Completes all documentation related to nursing and maintains accurate records, as required.
· Follows the plan of care, observes and reports any substantial findings and/or changes in a resident's behaviour, physical condition and/or food/fluid intake to the Director of Care/designate.
· Works in accordance with established job routines and schedules, as required.
· Supports with on-boarding of new hires, students and volunteers, as required.
· Participates in in-services and education sessions, as required.
· Assists with resident admittances, transfers, departures and deaths.
· Develops and maintains a resident assessment schedule for all InterRAI LTCF users and coordinates the completion of the InterRAI LTCF assessments for all residents, as required.
· Generates InterRAI LTCF reports to support clinical and operational decision-making, as required.
· Completes monthly and quarterly InterRAI LTCF and POC audits, analyzes audit results and indicators; identifies and implements corrective actions in conjunction with the Director of Care, as required.
· Oversees the development and coordination of the data submission process.
· Ensures InterRAI LTCF assessments, resident assessment protocols and plan of care reviews are completed in a timely manner and in accordance with CIHI guidelines and MOHLTC legislation.
· Provides education, coaching and mentoring for the development of nursing staff, as it relates to InterRAI LTCF, across all shifts, as required.
· Works with InterRAI LTCF users to develop processes that ensure data accuracy, efficient data collection, assessment completion and data utilization; provides support for validation of coding.
· Integrates InterRAI LTCF assessments with existing clinical tools and provides leadership and guidance on best practices related to InterRAI LTCF.
· Ensures that plans of care are created, updated, maintained and kept current, as required.
· Implements RNAO best practices and guidelines in all aspects of nursing, as required.
· Ensures compliance with organizational standards, the collective agreement, as applicable and all relevant regulations, and/or legislative requirements and strategic initiatives.
· Participates as a member of the multi-disciplinary team, providing resident information to aid in the development of plans of care.
· Participates in multi-disciplinary care conference, the strategic planning process and other meetings as required.
· Acts as an advocate for the resident and promotes the Resident Bill of Rights.
· Performs other duties as assigned.
Qualifications:
· Current registration and a member in good standing as a Registered Practical Nurse with the provincial College of Nurses.
· A certificate in InterRAI LTCF CIHI Essentials (#1, 2 and 3) from an accredited provider.
· Two years of experience as a Registered Practical Nurse in the long-term care sector, with a focus on InterRAI LTCF would be considered an asset.
· Ability to read, write, comprehend and follow instructions in English.
· Sound knowledge of the Fixing Long-Term Care Homes Act 2021, Ontario Regulations 246/10.
· Excellent leadership, oral and written communication skills and strong relationship building abilities are required.
· Proficiency with Microsoft Office, Point Click Care and other computerized documentation systems.
· Satisfactory vulnerable sector check and TB test.
· Has a passion to promote person-centered care and work with the senior population.
