Job Description
In this community support role, as a Personal Support Worker (PSW) you will have scheduled visits to care for patients with palliative care needs as part of a palliative care team comprising of Nurses, PSW's and Allied Health members. You will work with patients of all ages with differing complexity of care and play a key role in helping patients navigate through their care journey.

What you'll do:
Assist patients with activities of daily living including skin, hair, and oral care along with bathing, toileting, & peri-care
Liaise with other members of the client's palliative care team
Prepare nutritious meals and snacks, including special diets, as determined by the care plan
Provide basic housekeeping tasks and companionship to help your patients enjoy a better quality of life
What you bring:
Valid Personal Support Worker Certificate
PSWs palliative care and PPS trained: Palliative Care Fundamentals & LEAP PSW Course (or will be trained within 3 months upon hire) preferred
Ability to demonstrate compassion and care when interacting with patients
Reliability and flexibility
Passion for making a difference in your community
Valid driver license and access to a vehicle to travel to and from patient's residences within a geographical area
