Role Overview
The CM-SW supports clients of AHFT through a shared care interdisciplinary team approach, while assuming primary responsibility for an assigned caseload of clients and by drawing on their regulated social work designation to offer specific skills in areas such as, but not necessarily limited to, individual and group counselling.

The CM-SW is responsible for providing services to Cota service users in a manner consistent with Cota's Health's Mission, Vision and Values.

About The Team
This Case Manager-Social Worker (CM-SW) is a member of Cota's Assertive Housing First Team (AHFT). AHFT is an interdisciplinary team of health care professionals comprised of a program psychiatrist, nurses, peer support specialists, addictions specialists, and mental health clinicians which provides support to adults (18+ years) who are homeless in the City of Toronto and are living with a serious mental illness. The interdisciplinary team works with individuals to find safe and affordable housing based on personal preferences and provides intensive interdisciplinary team supports to assist them in maintaining housing stability and their pursuit of personal recovery.

Responsibilities
The CM-SW's primary duties may involve, but not necessarily be limited to:

  • Assuming primary responsibilities for providing case management support to an assigned caseload of clients
  • Counselling and educating service users, families, and various support systems about severe and persistent mental illness.
  • Interviewing individuals, families and significant others to assess service users' social, emotional, physical and mental impairments, as well as financial needs.
  • Assisting service users in obtaining appropriate community resources for financial support, such as ODSP, Ontario Works, supportive housing, etc.
  • Leading group counselling sessions and plans psychosocial programming for service users in need of specific services.
  • Obtaining a thorough social history in a timely manner with each new service user and bringing relevant concerns to the attention of all team members.
  • Providing individualized psychoeducation for families about the service user's illness and the role of the family and other significant people in the therapeutic process.
  • Supporting service user self-determination in relation to legal matters (charges, diversion, probation, CTOs, SDMs, PGT- guardianship, POA, Mental Health Advance Directives and PHIPA).
  • Supporting administration of medication programs in collaboration with AHFT psychiatrist, including administration and delivery of medications
  • Sharing shift work, including weekends with other AHFT Staff is also required.

  • Adheres to all Cota's policies and procedures and follows established applicable program guidelines, procedures and practices.
  • Adheres to all applicable legislation, particularly the Ontario Human Rights Code.
  • Complies with all Health and Safety policies and procedures to ensure every reasonable precaution is taken for the protection of workers and clients and that health and safety concerns or incidents are responded to promptly.
  • Works cooperatively, respectfully and constructively with clients, Team members, management, other service providers, and external stakeholders.
  • Ensures that the confidentiality of clients, employees and Cota information is always maintained.
  • Undertakes other related and/or delegated duties, as assigned.

Qualifications
Education:

  • Minimum of a Bachelor of Social Work (BSW) degree and current registration as a Registered Social Worker through the Ontario College of Social Workers and Social Service Workers.

Experience

  • 2 years' experience working with adults living with serious mental illness
  • Experience working in the community

Knowledge And Skills

  • Excellent clinical knowledge relevant to the target population being served and/or unique role, as demonstrated through specialized training, or previous work experience.
  • Excellent clinical assessment/support skills relating to the specific target population.
  • Sound knowledge of community resources relevant to the target population being served and/or demonstrated ability to search for applicable resources/information.
  • Excellent interpersonal and communication skills (verbal and written). High level of computer literacy in relation to systems/applications used in the role (Word, E-mail, Internet).
  • Good judgment and ability to work independently as well as cooperatively with others.
  • Ability to solve problems and make recommendations to resolve issues.
  • Effective team player.
  • Sensitive to challenges experienced by marginalized population groups such as poverty, access issues, mental health and addictions.

Other

  • Flexibility and willingness to work varied 'regular' work patterns to suit the needs of the program, however typically Monday to Friday and two weekend days a month.
  • Must have access to a vehicle. (Please note that access to a vehicle means owning, borrowing, or renting the vehicle. Alternatively, using ride services, such as Uber, cab, and taxis at no cost to Cota).

Upon successful completion of a three-month period of employment, permanent full-time and permanent part-time staff working more than 20 hours per week will be eligible for employer-paid group insurance benefits. These employer-paid benefits will include:
