Hours: As scheduled (Monday to Friday, 8:30AM - 4:00PM, excludes statutory holidays)

JOB SUMMARY

The Licensed Practical Nurse (LPN) duties is within the competency guidelines and standards of practice outlined by the British Columbia College of Nurses and Midwives (BCNNM), the current defined scope of practice and employer policies within a community healthcare setting.

DUTIES & RESPONSIBILITIES

  1. The LPN assesses, plans, implements, and evaluates nursing care to meet the needs of clients in accordance with competency guidelines within the established BC College of Nurses & Midwives (BCCNM) standards of practice and employer policies within community healthcare setting.

  2. The LPN coordinates health services and provides support in activities of daily living, including medication a reminding

  3. The LPN overseas the referrals process and assists clients with acquiring transportation to program activities.

  4. The LPN informs Executive Director/designate and Case Managers when client's condition is no longer stable and predictable, or the care is outside the LPN's scope of practice.

  5. The LPN provides safety and health information to individuals and their families, shares work related information with others, demonstrates clinical tasks and provides guidance to others.

6.. The LPN provides input regarding clients' needs, performance, and progress. Attends multidisciplinary care meetings.

  1. The LPN performsnursing care, procedures and evaluates outcomes by completing care plans.

  2. The LPN observes clients and their environments, and reports unsafe conditions and behavioral, physical and/or cognitive changes to Executive Director/designate.

  3. The LPN provides emotional support and annual feedback to clients and their families. Leads the caregiver support group.

  4. The LPN completes and maintains related records and documentation such as progress notes, observations, and client charting.

  5. The LPN answers general inquiries by telephone and in person, provides direction and routine information about programs and policies.

  6. The LPN attends staff meetings, in-service sessions, and staff development.

  7. The LPN performs other related duties as assigned that are within the competency guidelines and standards of practice.

QUALIFICATIONS

Education, Training and Experience

  • Graduation from a recognized program for Licensed Practical Nurses
  • Current full practicing licensure with BCCNM
  • Certificates in CPR and First Aid
  • Current immunization record

Skills and Abilities

  • Ability to execute Nursing process competently
  • Ability to observe, recognize and chart changes in clients
  • Ability to communicate with Case Managers and process referrals
  • Physical ability to carry out duties of the position
  • Ability to speak, read, and write fluently in English
  • Ability to converse in Cantonese and/or Mandarin is an asset
  • Ability to operate related equipment
  • Ability to work independently and in cooperation with others.
  • Ability to organize and prioritize
  • Ability to establish and maintain rapport with clients and family members

Job Type: Full-time

Pay: $32.84-$41.35 per hour
