We are a community health care agency that provides a full range of community health care and support services, making us a distinctive agency in Toronto offering a blend of community support services, community mental health and addictions services, supportive housing, inter-professional primary care and home care services. We are community governed. We continually look for ways to serve our clients and our community better and to provide enhanced access to equitable care. We are looking to expand our Primary Health Care team with the addition of a new Social Worker/Therapist.
Position Statement/Summary
As the social worker/therapist, you will provide counseling/therapy, case management, client advocacy and group work for children, youth, and adults, on and off-site, virtually and in-person, including homes, schools, shelters, and other community locations. You will work independently and as part of an interdisciplinary team, from framework that is trauma-informed, with a focus on anti-black and indigenous racism, discrimination, and oppression. Care practices also include a focus on the key determinants of health, with an emphasis on prevention, empowerment, health promotion and community capacity building. This is a unionized position.
Key Responsibilities
Assess the psycho-social needs of children, youth, families, adults and seniors and work with them to develop and implement meaningful and attainable plans/goals.
Provide counseling and therapy related to trauma, mental health, self-harm, disordered eating, eating disorders and disturbances, addictions, sexual health, family conflicts, school conflicts and other issues.
Provide comprehensive care to populations facing multiple barriers, ensuring access to those most vulnerable.
Provide crisis intervention and suicide intervention.
As the mindfulness expert, maintains a mindfulness practice.
Advocate and promote linkages and access to services that meet the needs of children, youth, families and seniors.
Provide itinerant services working from different locations to create greater access to populations facing barriers to care.
Perform home, school, shelter and hospital visits, as required.
Support and participate in case management and care coordination functions in collaboration with the interdisciplinary team.
Conduct outreach and plans/ participates in community engagement measures.
Participates in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services for individuals to foster self-help, mutual aid, self-determination, and capacity building.
Incorporate and strengthen collaborative and interdisciplinary work.
Contribute to programs and services cycle of planning and evaluation, and participate in center-wide planning, committees, and staff meetings as appropriate.
Participates in inter-professional research with colleagues - internal and external to the Centre, as appropriate.
Identify areas where the development of protocols and procedures is needed to improve client services or to promote more effective staff functioning.
Develop protocols and procedures as required and ensure that services are evaluated.
Develop partnerships and alliances to optimize health care service provision and address needs of community and participates in community groups and committees relating to the development and delivery of health care and social services in the community.
Contribute to the Centre's activities to collect, analyze and report on data and relevant information, and participate in research when appropriate.
Maintain and develop professional competence through appropriate continuing education methods.
Support the Centre's student and volunteer placement programs, and supervises students and volunteers as required.
Perform duties assigned by the supervisor.
Qualifications
· Graduate degree in Social Work from a recognized university or an equivalent degree certification with the Ontario College of Social Workers and Social Service Workers or the College of Registered Psychotherapy of Ontario.
· Minimum 3-5 years counseling and therapy experience, preferably in a community-based human services organization.
· Experience providing comprehensive care to populations facing multiple barriers and with comorbidities, including mental health and addictions.
· Additional supervised clinical training.
· Training in mindfulness and having mindfulness practice is an asset.
· Training in Eye Movement Desensitization Reprocessing (EMDR) and DBT is an asset.
· Thorough knowledge of and proficiency on current assessments, and psycho-social counseling techniques with children, youth, adults, seniors, families, couples.
· Demonstrated experience in crisis prevention and de-escalation.
· Demonstrated ability, commitment to and knowledge of community health care.
· Demonstrated ability to work in a team-based model and within an inter-professional team environment.
· Extensive experience working from a framework that is trauma informed, with a focus on anti-black and indigenous racism, discrimination and oppression and ensuring a positive space.
· Experience in program development, implementation, monitoring, and evaluation for therapeutic groups.
· Excellent interpersonal skills with clients in a culturally diverse practice population, and with colleagues.
· Excellent group facilitation skills.
· Excellent verbal and written communication, decision-making, problem-solving, conflict management and time management skills.
· Proficiency in word processing, use of computers, and various software applications, including computerized clinical information systems software.
· Ability to speak a second language, relevant to catchment area populations (Spanish, Portuguese, Polish, Ukrainian, Russian, Arabic, Tigrinya), is an asset.
