The Patient Flow Navigator will support the philosophy that each patient receives the right level of care, in the right place, at the right time by the right provider. Responsibilities of this position include but are not limited to:
- Addressing identified processes and/or other issues impeding the patient's flow from admission to discharge.
- Coordinating the multidisciplinary team's plan of care to make sure needs are met promptly, and ensuring consistent messaging to the patient and family regarding discharge plans is ensured
- System navigation and bed placement
- Building awareness of the patient navigator role/services among the health care team is important to assist in coordinating patient care and identifying "at-risk" patients that need navigation services.
Qualifications
Certification/Experience/Knowledge:
- Registered and a member in good standing with the College of Nurses of Ontario is required
- Minimum of two (2) years of recent (within the last 2 years) clinical experience working within the patient flow department
- Demonstrated knowledge of various bed types within NBRHC and off-service admission criteria.
- Ability to make bed placement recommendations to bed allocation based on patients' care needs and admission criteria to each unit. (i.e. Surgery/Medicine, Complex Continuing Care, Rehabilitation, CCU, Mental Health, ACU, SSSU, Peds, D2, E3 etc.)
- Knowledge of patient flow tools, such as Estimated Discharge Date (EDD) and patient flow dashboard
- Knowledge of community resources and programs required
- Demonstrated ability to work collaboratively in a multidisciplinary team environment, including internal and external partners required
- Experience with the repatriation process, inter-facility transfers, and direct admission
- Exceptional understanding of ALC, knowledge of ALC designations, discharge destination detail, most appropriate discharge destination, special needs, and barriers.
- Practical knowledge of relevant legislation required
- Current Non-Violent Crisis Intervention (NVCI) Certificate required (training delivered by Health Centre upon hire)
- Must demonstrate commitment to ensure your safety, the safety of fellow workers, patients, volunteers, and visitors
- Must support and contribute to a culture of safety and prevention of adverse health events in this organization
Skills/Abilities:
- Demonstrated familiarity with discharge planning principles and resource utilization required
- Demonstrated strong leadership, critical thinking, conflict resolution skills, and problem-solving skills required
- Demonstrated exceptional organizational skills required
- Excellent communication and interpersonal skills
- Self-directed professional who functions with minimal supervision and can rapidly shift workload priorities
- Ability to support continuous quality improvement initiatives
- Demonstrated commitment to ongoing education is required
- Good attendance and work record required
- Bilingualism (Advanced level English/French) is an asset.
