JOB SUMMARY: The Personal Support Worker reports to and works under the direction of the Case Manager of the Case Management Unit assigned to their work area within the Home and Community Care Program under the Six Nations Wellbeing Department.
To assist the Wellbeing Department and other Personal Support Workers in the performance of statutory, operational and advisory services duties related to the Six Nations Home and Community Care Program; within the policies and procedures established by the Six Nations of the Grand River Elected Council for the Six Nations Home and Community Care Program.
To assist the Case Manager of the Case Management Unit assigned to their work area by providing home care services for individuals and families of Six Nations of the Grand River Elected Council Territory during periods of illness, incapacitation, convalescence, or family disruption

BASIC QUALIFICATIONS:
- Personal Support Worker Certificate
- Must be a minimum of Grade 10
- Must have a vehicle and valid Class "G" driver's license
- Must be willing to work 12-hour shifts

POSITION TITLE: Jay Silverheels Complex (JSC) Personal Support Worker
REPORTING RELATIONSHIP:
Reports to and works under the direction of the Case Manager of the Case Management Unit assigned to their work area within the Home and Community Care Program under the Six Nations Wellbeing Department.
PURPOSE & SCOPE OF THE POSITION:
To assist the Wellbeing Department and other Personal Support Workers in the performance of statutory, operational and advisory services duties related to the Six Nations Home and Community Care Program; within the policies and procedures established by the Six Nations of the Grand River Elected Council for the Six Nations Home and Community Care Program.
To assist the Case Manager of the Case Management Unit assigned to their work area by providing home care services for individuals and families of Six Nations of the Grand River Elected Council Territory during periods of illness, incapacitation, convalescence, or family disruption.
KEY DUTIES & RESPONSIBILITIES:
Technical Functions –
- Functioning in accordance with current standards, policies and procedures, and practices of the Six Nations Home and Community Care Program.
- Assuming responsibility for skill improvement and self-growth by attending orientation and mandatory in-service programs.
- Providing a stable safe environment while maintaining the client's activities of daily living by:
- Assisting with personal care. Ex. Grooming, dressing, toileting, feeding, encouragement to take medications, and mobility assistance. Any other task delegated by a registered nurse according to the College of Nurses of Ontario guidelines for decision-making and working with Unregulated Care Providers (UCP's)
- Performing a variety of household duties. Light housekeeping, sweeping, mopping and laundry
- Planning and preparing nutritionally sound meals, including special diets as required by the treatment plan in consultation with a registered dietitian.
- Maintaining proper use of client charts as follows:
- Observing and reporting pertinent data related to the client and their home environment to the Case Manager on a regular basis or as necessary
- Reporting and discussing any requests for changes in the treatment plan by the client or the client's family to the Case Manager/Complex Care Nurse
- Providing instructions regarding the activities of daily living (ADL's) via demonstrations and written direction in the client's treatment plan
- Cooperating with all members of the Six Nations Home and Community Care Program team in the delivery of client care.
- Working with other health care team members. Nurses, Physiotherapists, etc. to assist in achieving the treatment goals.
Administrative Functions
- A current client record is maintained on each client that:
- Contains up-to-date Personal Support Worker's notation relating to the Care Plan/Treatment Plan
- Contains up-to-date Personal Support Worker's Activity Record with initials to account for completed tasks
Communications Functions
- Communicating with other team members regarding the client-related issues by:
- Maintaining confidentiality of information regarding the client, family and Six Nations Home and Community Care Program.
Other Functions
- Presents a positive image of the Six Nations of the Grand River Elected Council as follows:
- Presenting self in an acceptable, professional manner.
- Uniform or dress clean, neat and tidy.
- Hair well groomed.
- Good personal hygiene.
- Safe and appropriate foot ware.
- Wear name tag.
- Demonstrating a caring attitude toward clients, family members, other team members, and the job itself.
- Showing confidence in the role of a Personal Support Worker
- The safety of the client and their family is maintained by:
- Carrying out duties according to Health and Safety Standards for clients and their families, for self, and other personnel.
- Providing support for the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Director of Health Services as a member of the Emergency Control Group
- Performing other related duties as required by the Six Nations Home and Community Care Program Case Manager.
KNOWLEDGE AND SKILLS:
Minimum Requirements:
- Personal Support Worker Certificate
- Must be a minimum of Grade 10
- Must be in good physical health. Ability to bend, lift, walk, sit, carry, pull, kneel, push. Doctor's certificate required
- Must have a vehicle and a valid Class "G" driver's license
- Must be willing to work 12-hour shifts
- Current certificate in First Aid and CPR
- Must pass a criminal record check

IMPACT OF ERROR:
Errors in judgment and in the conduct of duties could lead to loss credibility, poor public relation, confusion, duplication of effort and misinformation being given to the Case Management Unit. Manager of the Six Nations Home & Community Care Program, Director of Health Services, Health Commission, Six Nations of the Grand River Elected Council, Government agencies and the public.
