Job Description: The Assessor Coordinator Degree is the primary coordinator of client admissions. The incumbent is responsible for review of community Mental Health and Addiction assessments, screening, consultation, data collection, organizing, communicating client admissions, client/family and community education. The incumbent will adhere to the current Provincial Clinical Addiction Model for practice and SHA philosophy, objectives and standards, be familiar with current drugs of abuse, concurrent disorders, and withdrawal management strategies. The incumbent will work with staff including but not limited to nursing, psychology, management, senior staff to triage acute medical, mental health, and detoxification concerns. The incumbent will work in collaboration with the Manager regarding program development to ensure continuous improvement occurs using evidence­ based information. The incumbent is responsible for providing case consultation, case assignment, and orientation. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Knowledge and Abilities

  • Ability to adapt to changing conditions
  • Trained in current ASIST suicide risk assessment model
  • Knowledge in addictions screening, assessment tools and procedures
  • Knowledge of provincial human service org, resources and referral procedures
  • Knowledge in Transtheoretical model of change
  • Applied knowledge of Suboxone/Methadone/Sublocade assisted recovery programs
  • Ability to teach & train a variety of users in workshop and individual sessions
  • Demonstrated high degree of accuracy is required of assigned responsibility
  • Knowledge in community resources and services in the clinical area
  • Knowledge in clinical assessment, diagnosis and treatment
  • Knowledge in principles, programs and practices in community mental health care
  • Applied knowledge of drugs abuse and their effects during use and withdrawal
  • Knowledge of Health Information Protection Act and other legislation.
  • Demonstrated knowledge of youth and adult Mental Health & Addiction treatments
  • Demonstrated knowledge in the area of assessment and referral coordination
  • Knowledge of how mental health impacts the individual, community and families 
  • Ability to complete case management assessments, follow-up and treatment
  • Has demonstrated strong interpersonal and communication skills.
  • Ability to work as a member of a multi-disciplinary team
  • Ability to plan/organize/coordinate with other multidisciplinary team members
  • Ability to make decisions in accordance with policies/procedures of SHA/MHAS/ED
  • Ability to prioritize to meet deadlines and work effectively under pressure 
  • Understands Mental Health Services Act, forms, policies & procedures of SHA/MHAS
  • Initiative in organizing work in accordance with departmental procedures
  • Knowledge in harm reduction in Addictions Medicine
  • Ability to work independently and in collaboration with others as required
  • Ability to clearly articulate assessment findings 
  • Ability to communicate effectively with all internal and external contacts.
  • Knowledge of motivational interviewing
