The Clinical Resource Program provides a centralized model of service delivery and coordination in the areas of Patient Flow, Staffing and Scheduling, and Clinical Resource Management. The Clinical Resource Program is accountable for the delivery of roles, resources and structures required to achieve clinical efficiencies across Hamilton Health Sciences.
- Current registration in good standing with the College of Occupational Therapists of Ontario (COTO)
- Experience working in an inter-professional team
- Knowledge of outcome measurement and evidence-based practice
- Demonstrated ability to work in a changing environment with high productivity, organizational and flexibility demands with solid clinical reasoning and caseload management skills
- Must have excellent communication and collaboration skills, and the ability to work within interdisciplinary teams across the hospital
- Familiarity with the Barthel Assessment Tool an asset
- Minimum of 3 years experience working with the geriatric population
- Demonstrated knowledge, skills and abilities to develop individualized coordinated care plans for seniors living with multiple chronic diseases, frailty, cognitive impairment etc.
- Ability to perform well in a fast-paced environment
- Demonstrated expertise in complex geriatric assessment and restorative approaches
- Demonstrated current knowledge of and experience with discharge planning and community resources
- Ability to creatively problem solve and facilitate change

The Occupational Therapist functions as a valuable member of the interdisciplinary team and provides care to frail, complex senior patients. The role has a dual focus\: to prevent avoidable admissions by liaising with community supports; and to provide early assessment and intervention associated with best practice for geriatric patients. The In addition, the Social worker will actively participate in continuing professional development opportunities, professional practice and program committees & initiatives that support patient care, clinical practice, education and research as approved by the manger. The OT will provide comprehensive screening and assessment related to 7 modalities of care and social needs for patients that are 65 and older. The OT will work collaboratively other members of the centralized care and transition team to provide early, interventions and to develop comprehensive care plans. Initial contact with patients will occur in the emergency department and the Express unit, and for patients with very complex needs related to transition planning, the OT will follow them throughout the entirety of the journey in acute care. The OT participates as a member of an Interprofessional team and provides direction and supervision to OTA/PTA. The Occupational Therapist (OT) practices in accordance with the standards and guidelines for professional practice as per the College of Occupational Therapists of Ontario (COTO) and according to Hamilton Health Sciences policies & procedures. The OT incorporates best available evidence into practice. In addition, the Occupational Therapist will actively participate in continuing professional development opportunities, professional practice and program committees & initiatives that support patient care, clinical practice, education and research as approved by the clinical manager.
