Job Description: Under the direction of the Program Manager responsible for Intake, and as part of an interdisciplinary team, the Assessor Coordinator is responsible to respond to initial requests for information and services across Mental Health and Addiction sites. The Assessor Coordinator will provide a range of screening, triage, assessment, treatment and case management services to a population of acute and chronic mentally disordered adults as well as children /youth and their families. The incumbent will provide program consultation and coordination of service including prioritizing and assigning to multidisciplinary staff or referring to community agencies . Also provides professional and public education regarding mental health issues and resources. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Education
- Recognized degree/diploma in nursing, or a degree in Social Work.
- Must have a current practicing registration with their professional association.
Experience
- Requires a minimum of one (1) year recent post graduate experience working within community mental health and addiction services, with combined experience working with adults, children/youth and their families in a health care service setting.
Knowledge, Skills and Abilities
Knowledge and skill in assessment, triage, treatment, case management, community outreach and crisis interventions. Knowledge of normal child and adolescent development and psychopathology. Knowledge of family systems and models for therapeutic intervention strategies appropriate to the treatment of child, adolescent mental health diagnoses and family problems. Knowledge of interventions empirically validated or supported by best practice literature. Knowledge of a variety of mental health disorders and addiction and harm reduction strategies required to enable client to sustain stability. Ability to identify, implement and evaluate areas of client difficulty across a wide rangy of clinical presentation and formulate evidence based intervention. Ability to implement a variety of intervention strategies (individual or group) for individuals with mental health and addiction problems in a community based setting, including but not limited to assessment, triage, treatment, case management. Knowledge and practical application of the Mental Health Services Act, the Family Services Act and the Health Information Privacy Act. Demonstrated cultural competency skills. Organizational skills, including the ability to maintain priorities while managing multiple and diverse demands. Leadership abilities and the ability to work within a change management model. Interpersonal competence, including effective problem solving and conflict resolutions skills. Knowledge of community resources and demonstrated ability to develop and maintain consulting relationships within a multidisciplinary team, interagency programs and across the community of human services. Ability to work collaboratively as a member of a multidisciplinary team, and independently to manage complex and multiple demands. Sound communication skills - both written and oral. Ability to prepare reports in a clear, concise and timely manner and ability to provide public presentations and professional training. Effective problem solving, decision-making and conflict resolution skills.
