As a Social Work Assistant with VCH you will:

• Work under the general operational supervision of the Patient Services Manager, clinical supervision from the Practice Coordinator and receive work direction from Social Workers in a team based environment and provides social work support services during the admission and discharge of patients/clients.
• Perform duties including: interviewing clients/families/facilities for information, assisting clients to approach external resources, providing practical support to clients regarding benefits, housing, transportation, community resources, finances, and acquiring and maintaining a supply of up to date information and applications on services/agencies/resources.

About Adanac Park Lodge (APL):

Adanac Park Lodge (APL) is a regional Vancouver Coastal Health (VCH) Long Term Care (LTC) program specializing in providing care to younger adults years) with complex health and behavioral needs.  APL's unique population of focus is younger adults who have concurrent Mental Health, Substance Use and/or Acquired Brain Injury; and who often have co-existing physical, developmental and/or chronic health conditions.  Clients who may be considered for admission are between19 and 65 year of age.  Residents may reside at APL for as long as they require the specialized services / environment to meet their care needs, up to an including end of life care.  APL has a diverse inter-disciplinary team who collaborate with residents and families in their care and on meaningful goals and engagement.  In accordance with the vision and values of the organization, APL's approaches & principles include; Person Centered Care; Trauma Informed Care; Recovery in MH; and Community Engagement.

Education & Experience

• Completion of a recognized Social Service Worker or Social Service Diploma Program or an equivalent combination of education, training and experience.

Knowledge, Skills & Abilities

• Ability to communicate effectively both verbally and in writing.
• Ability to deal with others effectively.
• Physical ability to carry out the duties of the position.
• Ability to prioritize and organize work.
• Ability to operate related equipment.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
