Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPNs) to manage our Community Care Division. In this role, you will provide quality client care, perform a variety of nursing tasks within your scope of practice, and collaborate with healthcare professionals to ensure optimal outcomes in a home care environment.

Key Responsibilities

  • Administer treatments as prescribed by physicians as well as work with the other nurses in the agency to ensure clients receive proper healthcare
  • Maintain accurate and timely client records and documentation
  • Collaborate with interdisciplinary healthcare teams as required
  • Complete monthly reports for funders
  • Support community health workers to ensure the highest standard of client care
  • Complete and submit montly reports to funders
  • Provide education as needed to the community health workers

Qualifications & Skills

  • Current registration with BCCNM as an LPN
  • Strong communication and interpersonal skills
  • Excellent time management and organizational abilities
  • Proficient in medical terminology and clinical documentation
  • Compassionate, client-focused approach to care
  • Managerial experience an asset
