Key Responsibilities

• Assess patient health through examinations and history reviews
• Administer medications/treatments and monitor responses
• Develop and implement personalized care plans in collaboration with teams
• Document accurately using EHR systems.
• Deliver specialized care in pediatrics, seniors, dementia, pain management, and procedures like phlebotomy
• Educate patients/families on health management and discharge planning
• Ensure regulatory compliance and a safe care environment

What We're Looking For

• RN / LPN licensure in good standing with CRNM, CLPNM.
• Active BLS.
• Experience in acute care, diverse populations (PCH, seniors, home care, etc.) preferred
• Proficiency in medical terminology, medication administration, and patient assessment
• Skills in case management, pain management, dementia care, and phlebotomy highly valued
• Strong communication, teamwork, and compassionate approach
