Acclaim Health announces the availability of one (1) 0.6 Established Part Time Hospice Counsellor position which is equivalent to 22.5 hours/week for the Community Support Services Department to work in the Halton area with a focus on Milton and Halton Hills community. The individual will be required to work 3 shifts per week from 8:30am - 5:00pm.

**_ Pay: $56,238 - $59,631 per year_**

**PRIMARY RESPONSIBLITIES**:

- Provides support to clients facing a life limiting illness and their caregivers
- Conducts initial client assessments to determine eligibility and to assess specific clients’ needs.
- Works with the client and caregiver (if applicable) to develop a care plan to meet needs identified.
- Provides comprehensive and culturally sensitive psychosocial support to clients and caregivers
- Matches and assigns volunteers to clients and monitors their relationship
- Shares information and education with clients and caregivers
- Provides referrals to other community resources as needed
- Attends palliative team meetings and participates in case conferences as required
- Provides consultation and education to community partners

**QUALIFICATIONS, EDUCATION**:

- Degree in social work, nursing or equivalent
- Training in hospice palliative care
- Certification in CPR-Basic Rescuer Level
- Valid Ontario driver’s license, use of vehicle and insurance in accordance with the Acclaim Health policy

**EXPERIENCE & SKILLS**:

- Minimum 2-4 years demonstrated experience working in palliative care
- Strong understanding of hospice philosophy of care.
- Demonstrated ability to connect to and relate with children and youth
- High level of comfort engaging with others in end-of-life discussions and bereavement experiences
- Understanding of mental health concerns
- Ability to provide culturally safe support
- Strong verbal, written communication and interpersonal skills
- Strong listening, assessment and conflict resolution skills
- Effective decision-making, problem-solving and organizational skills
- Proven ability to multi-task and meet deadlines
- Proficient in Microsoft Office, AlayaCare.
- Demonstrated ability to work independently and collaboratively as a team member.
