**About the Opportunity**:
The Placement Coordinator is responsible for supporting the process of Placement in a Long Term Care Facility on behalf of the Nova Scotia Health. Reporting to the Provincial Manager, Continuing Care Placement, the Placement Coordinator liaises with home care, acute care, and long term care facilities to facilitate appropriate and effective long term care placements. The Placement Coordinator receives long term care assessments for clients who are referred for Placement, communicates with client/Substitute Decision Maker on placement issues, manages the waitlist for Long Term Care ensuring that clients are admitted to long term care as per the Department of Seniors and Long Term Care policy, coordinates admissions to Long Term Care facilities; coordinates transfers of residents between facilities; maintains records and provides written and statistical reports as identified by Nova Scotia Health. The Coordinator also serves as a representative for Continuing Care with the public on matters related to admission to long term care facilities.

**About You**:
We would love to hear from you if you have the following:

- Bachelor’s Degree in a health related discipline/field
- Must be currently registered or eligible for registration with applicable professional association.
- Minimum 5 years clinical experience
- Two
- three years’ experience in the Continuing Care environment preferred
- Proficiency in Microsoft Office programs (Word, Excel, Outlook, and Internet) required
- Computer proficiency
- Experience in long term care assessment/placement would be an asset
- Knowledge of the health system
- Strong client focus and ability to build solid client relationships
- Demonstrated commitment to customer service

**Hours**:

- Permanent Full-time Position; 70 hours bi-weekly

**Compensation and Incentives**:
$32.82 - $41.44 Hourly
