The Extensive Needs Service is a specialized program that provides clinical services to children and youth with extensive needs whose needs cannot be met in a single service stream and who require an integrated approach across Mental, Behavioural and Developmental Health. The Extensive Needs Service is jointly funded through The Ministry of Health and The Ministry of Children, Community and Social Services and implemented jointly through Holland Bloorview Kids Rehabilitation Hospital, Children’s Hospital of Eastern Ontario (CHEO), and McMaster Children’s Hospital (MCH).

Holland Bloorview will provide Extensive Needs Services (ENS) across the greater Toronto area - a region comprised of multiple cities with many culturally and economically diverse communities. To promote equitable access to ENS and with a commitment to provide services that best meet the needs of unique communities, Holland Bloorview will work with system partners to deliver an integrated service delivery model across the region. Partner organizations that will provide ENS in their communities will work collaboratively with Holland Bloorview as the lead agency as well as across a network of ENS providers within the GTA to develop a thriving and integrated community of practice: The Regional Network of Extensive Needs Service Providers. Network partners will work collaboratively to share knowledge, expertise and resources. While each partner organization will design and operationalize a service model that reflects the unique landscape and needs of their community, all partners will share in guiding principles as established in the Extensive Needs Service Mission, Vision and Values and “north star” of providing evidence based, compassionate, trauma informed and individualized care to children, youth and families with extensive and unmet needs.

Working closely with the Director of the Extensive Needs Service, the Clinical Implementation Lead (CIL) will play an integral role in the development, implementation and growth of the regional network. The CIL will support partner sites as they integrate into the ENS network, and will work collaboratively with key partner stakeholders to implement a model of care that aligns with the model established at Holland Bloorview.

As partner sites begin implementation of ENS within their own organizations, the CIL, in collaboration with the Director, will design, coordinate and lead regular and structured communication opportunities for shared knowledge translation between partner organizations with a goal of enhancing health system coordination, collaboration and resource sharing.

This role will primarily be located at Holland Bloorview but will involve travel to partner organizations across the GTA.

**KEY RESPONSIBILITIES**

**Development and Implementation of Extensive Needs Services**:

- Work collaboratively and closely with partnership agencies as they design, implement and evaluate ENS within their organization. This will include travel to partner sites on a regular basis.
- Participation on Joint Oversight Committee with each partner organization.
- Develop expertise in ENS model of care and provide guidance to partner organizations to ensure fidelity of ENS across the region.
- Make recommendations to partner organizations on staffing/professional mix based on volume targets, availability of resources and identified community needs.
- Participate in recruitment and orientation efforts of partner organizations, e.g. sit on interview panel for key roles, develop an ENS orientation manual, etc.
- In collaboration with research and evaluation team, support the implementation of a robust evaluation and outcomes measurement process at all partner organizations.
- Provide onsite and virtual support and clinical guidance to partner sites in all stages of development, implementation and evaluation. Be a clinical resource in program design and implementation.
- Oversee database development and data management.
- Determine and execute approved marketing/promotional strategies as required.

**Knowledge Translation and Development of a Regional Network**
- In collaboration with Director and partner organization key stakeholders, develop, implement and co-lead a community of practice of Extensive Needs Service providers across the region.
- In collaboration with Director, develop frames of reference, processes and standards for regular knowledge translation, learning and development opportunities.
- Develop standardized patient case presentation submission forms for partner organizations.
- Work with other leaders within Holland Bloorview to develop research opportunities within the areas of knowledge translation, provider experience and learning environment best practices.
