**About the Opportunity**:
Reporting to the Manager of Child and Adolescent Services, the Case Manager is responsible for the delivery of community-based case management, counseling and behavioral support services to children and adolescents aged 0 to 19 with severe disruptive behaviour disorders (and severe and persistent mental illness) and their families. The Case Manager works in conjunction with the multi-disciplinary team and under the supervision of the primary therapist (Psychologist/MSW).

The Case Manager supports the implementation of the treatment plan by providing regular, ongoing support to children and families in order to address behavioral, mental health, and substance use concerns by coordinating and adjusting service delivery in a manner consistent with the goals. The Case Manager must be able to work collaboratively and cooperatively as part of a multi-disciplinary team and is able to work independently within a flexible work schedule and environment in a manner consistent with the standards of the Nova Scotia Association of Social Workers and the NSH. Travel within the Cape Breton area is required. Evening hours is required.

**About You**:
We would love to hear from you if you have the following:

- Bachelors degree in Social Work from an accredited university
- Registered or eligible for registration with the Nova Scotia Association of Social Workers
- A minimum of 3 years experience working with children and adolescents with mental health, substance use, or concurrent disorders in a community based setting providing case management and other supportive interventions
- Experience and ability to work collaboratively as part of a multidisciplinary team within a flexible work schedule and environment
- Has demonstrated the ability to develop effective linkages with other health, social service and education agencies as appropriate.
- Experience implementing plans that use positive behavior management strategies to promote positive adaptive behaviors, independence and skills development for children and adolescents
- Demonstrated knowledge of evidence based standards and practices for behavioral intervention, parenting supports and case management for children and adolescents
- Meets with children and their families/caretakers, within the community (i.e., school, home) for
- the purpose of developing goals and strategies to address the assessed behavioral/developmental or mental health concerns.
- Facilitates the monitoring of outcomes relevant to the treatment goals.
- Valid driver’s license
- Competencies in other languages an asset, French Preferred

**Hours**:

- Permanent Full-time (100%) Position; 75.00 Hours Bi-weekly.
- Travel within the Cape Breton area is required.
- Evening hours are required.

**Compensation and Incentives**:
$ 33.80 - $42.68 hourly
