**THE FULL TIME SOCIAL WORKER OPPORTUNITY**
Join our dynamic team at GBGH As a vital member of our Allied Health team, you'll collaborate closely with Patient Care Managers and interdisciplinary teams under the guidance of the Manager of Allied Health. Your primary role will involve offering expert discharge planning support and consultative services directly to our valued patients.

**KEY ACCOUNTABILITIES & RESPONSIBILITIES**
- Engage with a diverse patient population spanning various areas, including inpatient acute medical, ICU, Surgical, Emergency, Obstetrics, and Geriatric Patients.
- Conduct comprehensive psychosocial assessments, crisis interventions, discharge planning, and counseling sessions for patients and their families.
- Serve as a liaison, offering invaluable assistance to patients and staff in accessing community resources.
- Manage discharge planning and patient flow seamlessly across acute and community sectors.
- Collaborate effectively as a vital member of the healthcare team, demonstrating flexibility in caseload and program assignments.
- Demonstrate adeptness in managing and prioritizing patient caseload within a bustling acute care setting.
- Ensure meticulous completion of patient documentation, workload measurements, and data requirements in alignment with Hospital, Ministry of Health, CLHIN, and departmental standards.
- Respond promptly and proficiently to clinical crises, leveraging your expertise as a resource within the treatment team.
- Uphold a culture of safety for oneself, clients, colleagues, and the work environment.
- Engage in reflective practice in accordance with College standards, fostering ongoing professional development and excellence.

Job Requirements:
**QUALIFICATIONS & EXPERIENCE**
- Bachelor of Social Work
- Master of Social Work Degree (M.S.W.) from an accredited University School of Social Work (preferred)
- Current registration with the Ontario College of Social Workers and Social Service Workers required
- Minimum one-year recent relevant experience in acute-in-patient hospital care or non-acute institutional setting preferred
- Excellent working knowledge of related provincial legislation (e.g., Mental Health Act, Regulated Health Act, Health Care Consent Act)
- Familiarity with community, regional, and provincial resources
- Experience in computerized environments with sound understanding of operations
- Understanding of Home First philosophy and ALC legislation/processes
- Demonstrated ability in individual, group, and family counseling
- Proven ability to engage hard-to-reach and marginalized populations empathically
- Ability to work independently and in team settings
- Experience with Meditech Expanse preferred, with proficiency in case note documentation
- Strong interpersonal, verbal, and written communication skills
- Proficiency in English and French considered an asset

**HOURS OF WORK**
Permanent Full Time. Must be available to work shifts including days, evenings, nights and weekends. Shifts and hours may change according to departmental requirements and as per the OPSEU collective agreement.

- Hourly rate of pay $39.16 - $50.77 hourly Bachelor of Social Work or $44.67 - $57.90 hourly Master of Social Work, plus a generous vacation allotment in accordance with the OPSEU Collective Agreement
