**As our new Resident Support Coordinator, you will**:

- Have a degree in Social Work or related field such as Social Service Worker with a speciality in gerontology
- Registered with the Ontario College of Social Workers & Social Service Workers
- Coordinate and support the transition of residents moving into a health care setting
- Support resident and family meetings, care conferences and therapeutic groups
- Provide education, training, coaching and counselling for residents, families and team members regarding mental health and well-being
- Support residents on the waitlist and ongoing assessment needs with residents and those waiting for services

**It would be an asset if our new Resident Support Coordinator has**:

- Current Heart Save Level of CPR
- Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Fixing Long-Term Care Act, 2021 and Health & Safety Act of Ontario
- Demonstrated experience working with seniors and/or persons with cognitive or physical impairments

You will work as key members of our Village and neighbourhood multidisciplinary teams to understand residents’ needs, and advocate for residents to actively participate in decisions about their physical care.
