**JOB** SUMMARY**: The **Diabetic Foot Care RPN**reports to and works under the direction of the Clinical Supervisor, Diabetes Wellness Program, under the Primary Care Services of the Six Nations of the Grand River Elected Council. Receives clinical guidance and support as required from appropriate health leadership.

The Diabetic Foot Care Nurse (RPN) provides specialized clinical services aimed at the prevention and management of foot-related complications associated with diabetes and chronic disease. As an integral member of the Diabetes Wellness Program (DWP), the RPN delivers safe, culturally grounded foot care, risk assessments, and client education in alignment with the standards of the College of Nurses of Ontario, as well as the policies of the Diabetes Wellness Program, Six Nations Department of Well-Being, and the Six Nations of the Grand River Elected Council.

This role plays a vital part in supporting limb preservation and early intervention strategies, while promoting client self-management, education, and prevention. Care is provided independently and collaboratively within an interdisciplinary team model, with an emphasis on improving access to foot care services for individuals living with diabetes or those at high risk of developing it. The RPN is expected to reinforce the values and philosophy of the Diabetes Wellness Program in all aspects of their work, beginning from the initial point of client contact.

**BASIC QUALIFICATIONS**:

- Registered Practical Nurse with a Diploma in Practical Nursing. Member of the College of Nurses of Ontario with a current license in good standing.
- Must have a current first aid and cardio-pulmonary resuscitation certificate.
- Must be willing to engage in cultural safety training.
- Added skills of foot care and diabetes education training.
- Added skills and training Immunization certificate, Community Health Nursing Program, and Computer training, are assets.
