**Position Profile**:
This full time Contract position provides mobile case management services for previously homeless clients to enable them to stabilize their housing situation and live independently. The Housing Follow-up Support Worker works as an important member of our Breaking Barriers Program Team.

**Key Responsibilities**:

- Complete intake with clients, including providing program description and required documentation with respect and compassion
- Adopt housing-focused approach to prioritize harm reduction and support clients in recovery and housing stability
- Carry out home visits to clients who are at significant risk of homeless
- Provide case management planning, supportive counselling, crisis intervention, harm reduction, health and wellness resources and referral to other support services.
- Advocate on behalf of clients with Landlords, OW, ODSP, and other services.
- Assist clients to establish or maintain links with appropriate services including services related to general health care, addiction treatment, mental health care, identification/document replacement, volunteer opportunities, job training, and job placement.
- Provide life skills training in areas including landlord-tenant rights and responsibilities, budgeting, housekeeping, social integration etc.
- Ensure the delivery of services in a manner that prioritizes the personal safety of both the client and the case worker.
- Build positive relationships with landlords and clients of the program and stay informed of rental payments
- Ensure the confidentiality of all records, case notes, and files in strict accordance with the privacy legislation that governs the organization.
- Maintain accurate, comprehensive and timely activity records for each client, including detailed case notes and regularly updated care plans.
- Exhibits effective time management skills by setting priorities, creating a work schedule, monitoring progress towards goals, and effectively tracking details, data, information, and activities.
- Participate actively in team meetings and contribute to problem-solving, progress reporting, documentation and performance planning, to achieve the highest standards of program delivery.
- Support cross-departmental collaboration, contribute with enthusiasm to agency initiatives, and embody CONC values
- Contribute to a positive working environment.

**Qualifications**:

- College Diploma or University Degree in social work, mental health related field, or equivalent combination of education and work experience in a related environment
- Min. 2 years delivering client-centered support services and directly supporting vulnerable community members
- Completed training in de-escalation techniques and non-violent crisis intervention
- Ability to analyze and problem-solve, seeking input as necessary
- Understanding of diverse perspectives and ability to share information
- Understanding of strategies to work with vulnerable populations
- Some experience working with Indigenous communities
- Knowledge of conflict resolution strategies
- First-Aid, WHIMIS, CPI, Mental Health/Wellness, Trauma-informed Care, Harm Reduction
- Experience with addictions, developmental challenges & homelessness (preferred)
- Experience in case management & working with vulnerable/marginalized individuals
- Knowledge of community resources including substance abuse programs and other support services for homeless communities
- Ability to build collegial and effective professional relationships
- Skilled in organization of work, time management, communication, MS Office
- Current criminal record check

Pay: $46,332.00 per year
