HOURS: ALL SHIFTS AS REQUIRED

SALARY: $39.16 (MIN) TO $50.77 (MAX)

**QUALIFICATIONS**

**Certification/Experience/Knowledge**:
Registered and a member in good standing with Ontario College of Certified Social Workers and
Social Service Workers. Bachelor of Social Work (BSW) required, Masters in Social Work (MSW)
an asset

Current Non-Violent Crisis Intervention (NVCI) Certificate required (training delivered by Health
Centre upon hire)

Minimum 3 years of recent (within the last 2 years) experience with adult and paediatric populations
in an acute care setting (ED, CCU. Surgery, Medicine, Rehabilitation, Renal, Diabetes Center, and
Women and Children) with coordination of community services required.

Practical knowledge of relevant legislation required Knowledge of adult teaching and learning
principals combined with recent experience in facilitating groups required

Knowledge of community resources is required
A willingness to travel; a vehicle and valid driver’s license required

**Skills/Abilities**:
Excellent interpersonal, interviewing, and counselling skills required.
Psychosocial assessment skills and ability to complete reports in a timely manner required
Demonstrates strong case management skills and collaborates effectively with the interdisciplinary

team

Demonstrated familiarity with discharge planning principles and resource utilization required
Self-Directed professional who functions with mínimal supervision required.
Demonstrated commitment to on-going education is required
Must demonstrate commitment to ensure your safety, the safety of fellow workers, patients,

volunteers, and visitors

Must support and contribute to a culture of safety and prevention of adverse health events in this
organization

Good attendance and work record required
