**Job Summary**
The Manager, Community Programs, is a key leader at Southlake, responsible for the operations, budgeting, coordination, and strategic direction of all patient care and staffing functions within the assigned community program services. This includes oversight of patient care delivery and staffing, with all interdisciplinary team members reporting directly to this role.
The Manager leads with a patient-centred, evidence-based approach, supporting professional practice and fostering high-quality care. This role also requires strong fiscal and business acumen and serves as a role model for Southlake’s core values and competencies. The portfolio includes services for diverse populations such as long-term care residents, community-based palliative patients, and individuals accessing non-bedded hospital services.
**Skills & Abilities**
- Bachelor's degree in a registered health profession, master's degree preferred
- Registration with the appropriate College/Regulatory body
- Minimum of 5 years’ formal people leadership experience
**Qualifications**
- Minimum of five (5) years of progressive leadership experience, ideally in community and clinic-based healthcare settings, including experience leading teams in unionized environments.

- Demonstrated commitment to equity, diversity, and inclusion, with a leadership style that actively values different perspectives and lived experiences.

- Experienced in supporting performance and attendance management in a fair, consistent, and respectful manner.
- Academically and clinically prepared, with a solid foundation in evidence-informed practice.
- Exceptional interpersonal and communication skills, with a track record of cultivating positive and productive work environments.
- Demonstrated ability to lead through change using formal change management strategies and a people-first approach.
- Strong analytical, critical thinking, and decision-making skills to guide service delivery, planning, and problem-solving.
- Experience in quality improvement, research, or service design initiatives that improve patient and team outcomes.
- Actively engaged in committee work and stakeholder collaboration, with a focus on implementing and sustaining meaningful outcomes.
