Patient Safety Leader, Quality

**PURPOSE OF POSITION**
Reporting to the Manager of the Quality, Recovery and Patient Experience Department, the Patient Safety Leader is responsible for the day-to-day general operations management within the Patient Safety program as assigned. In conjunction with the Manager, the Patient Safety Leader will use clinical experience, knowledge and leadership skills to implement and promote best practice patient safety initiatives throughout the organization to mitigate unsafe acts and promote optimal patient safety outcomes through the use of evidence-based patient safety strategies and tools.
**KEY DUTIES AND RESPONSIBILITIES**
- Be knowledgeable of Patient Safety Required Organizational Practices (ROPs) applicable to inpatient and outpatient mental health clinical services;
- Assess organizational compliance with ROPs by collaborating with internal stakeholders;
- Maintain ROP Gap Analysis Document to reflect action plans in place to establish/enhance compliance
- Develop and operationalize to the Patient Safety Strategic Plan and implement patient safety initiatives, programs and processes, to facilitate corporate-wide compliance with patient safety-related Statutory, Regulatory, and Accreditation requirements;
- Provide patient safety consultation for unique and recurring patient incidents when needed;
- Work collaboratively with Professional Practice and the Enterprise Risk Management Department to foster coordination of practice initiatives with patient safety activities.
- Notify and support Clinical Managers, Clinical Nurse Specialists and Advanced Practice Clinicians when a trend is identified for unique patients to foster identification of patient safety issues and incorporating safety strategies in the Plan of Care;
- In consultation with Clinical Managers and facilitation of targeted trending to drill down into problem areas specific to a clinical area;
- Share and provide data in a meaningful way to support decision-making by clinical team, managers, directors, other departments and committees/task forces;
- Review and make recommendations to the incident reporting system (QRM) Administrator (Director, Enterprise Risk Management) regarding QRM template changes, dictionaries, and access permissions;
- Provide patient safety-related education to staff in collaboration with Professional Practice (e.g. introduction to patient safety practices, incident reporting) at Orientation, and in department/unit areas responsive to specific education needs general (e.g. initiatives or processes to support ROP adherence);
- Provide patient safety-related education to patients and families;
- Coordinate and support the facilitation of Executive Patient Safety WalkRounds;
- Facilitate or coordinate the implementation of process/system prevention strategies;
- Identify local and corporate patient safety indicators relevant to patient safety improvement opportunities;
- Participate in the identification and development of program/service, and organizational patient safety indicators;
- Responsible to recognize and report patient, staff, and environment safety issues, identify and implement preventative and corrective actions, within the scope of this role.
- Qualifications Required and Preferred**QUALIFICATIONS**
- Master’s degree in Health Science or Health Administration;
- Regulated Health Professional with current Regulatory College registration in good standing;
- Minimum 5 to 7 years clinical experience in a healthcare environment;
- Minimum 2 years working experience in healthcare patient safety, healthcare risk management, quality or professional practice;
- 2 to 3 years’ experience working in a mental health environment;
- Formal training and certification in patient safety, risk management and/or professional practice;
- Formal training in quality improvement;
- Willingness to actively pursue certification in Patient Safety, if not already attained.
- Working knowledge of professional practice standards, statutes and regulations governing the operation of a mental health public hospital and pertaining to patient care delivery and patient safety;
- Working knowledge of patient safety theory and patient safety tools used in a healthcare environment; expertise in root cause analysis, cause and effect analysis, failure mode and effects analysis; occurrence reporting, and disclosure of adverse events and/or unanticipated outcomes as applied to healthcare;
- Well-developed public speaking abilities and exemplary written and verbal communication skills, with the ability to establish positive collaborative working relationships with all staff, clients, families, community agencies and other professionals while employing tact and diplomacy;
