**Job Summary**

**Responsibilities**
- Provide personal care assistance to clients including bathing, dressing, and grooming.
- Administer medications as prescribed and monitor clients for any side effects or changes in condition.
- Assist with meal preparation and ensure proper nutrition for clients.
- Support clients in daily activities such as cleaning, laundry, and light housekeeping tasks.
- Engage clients in meaningful conversations and activities to promote mental stimulation.
- Assist with mobility and transfers, including heavy lifting when necessary.
- Maintain accurate records of client care and report any changes in health or behavior to the supervising nurse or case manager.
- Provide companionship and emotional support to clients, fostering a positive environment.
- Ensure compliance with medical terminology and protocols during caregiving tasks.

**Qualifications**
- Previous experience in nursing homes, senior care, or assisted living environments is preferred.
- Familiarity with working with individuals with disabilities and dementia care is highly desirable.
- Knowledge of medication administration procedures and patient care best practices is essential.
- Experience in home care settings is a plus, along with skills in meal preparation and cleaning tasks.
- Strong understanding of hospice care principles is beneficial for providing end-of-life support.
- Excellent communication skills and the ability to work collaboratively within a team setting.
- Ability to perform heavy lifting as required by client needs.
