The position of Housing & Program Initiatives Facilitators is responsible for championing new housing initiatives and opportunities that are adaptable and suitable to individual needs, community supports that promote teaching and learning, and financial supports for community based housing arrangements and strategies to support more independent community involvement. The Facilitator must have strong relations with government organizations that provide housing supports or initiatives, landlords, housing corporations, families, community members, and other service providers.

***MINIMUM QUALIFICATIONS**
- 2 year Community College diploma, preferably DSW, CYW
- 3 years’ practical experience with individuals who have a developmental disability in a community setting
- Knowledge and understanding of the needs and best practice service standards for persons with developmental disabilities
- Understanding of the impact of a developmental disability on the individual and family
- Good interpersonal skills, personal maturity and the ability to be a positive role model
- Excellent oral and written communication skills
- CPR, First Aid and pharmacology training
- Certification in non-violent crisis intervention (CPI)
- Valid Ontario driver’s license and access to a reliable vehicle

**DUTIES AND RESPONSIBILITIES**:
**1. **PROVIDES HOUSING SUPPORT SERVICES TO CLIENTS BY**:

- Identifying housing options or opportunities for individuals who wish to live more independently
- Developing a transition plan, identifying teaching and learning requirements and developing a transition plan with individual or individuals if a shared living arrangement is required
- Liaising with family, community supports, friends, and employers who wish to be part of the support process and identifying active support roles
- Identifying financial resources and supports required
- Exploring trial opportunities for independent living
- Identifying new or continuing community and social supports
- Identifying technological supports and other specialized requirements.

**2. PROVIDES COMMUNITY SUPPORT SERVICES TO CLIENT By**:

- Matching the community participation support with the new home environment.
- Designing and developing innovative programs aimed at achieving long and short-term goals, in accordance with the Individual’s Support Plan and living arrangement.
- Focusing of activities should be compatible with the individual’s interests and opportunities. Emphasis on providing a flexible array of meaningful teaching and learning opportunities while respecting personal choices.
- Evaluating instructional programs/activities and their impact on the individual and recommending necessary adjustments as required.

**3. **PROVIDES SUPPORT TO CLIENTS BY**:

- Functioning as primary resource for community participants.
- Initiating and maintaining contact with others involved in providing support to community participation (i.e. Case Coordinators, Social Workers, AYCC, Child and Family etc.).
- Emphasis is on sharing information and ensuring consistency in all environments.
- Providing an appropriate role model for daily living skills and good conduct.
- Developing annual Community Participation Service plans and case plans for an assigned group.

**4. PROVIDES ADMINISTRATIVE SUPPORT BY**:

- Providing the necessary documentation regarding the abilities, needs and interests of individuals supported.
- Providing input for annual Life Plans and ISP’s.
- Creating Annual Plan.
- Participating in team meeting and sharing information with other staff about specific situations as determined by the Manager.

**5. OTHER RELATED DUTIES MAY INCLUDE (BUT ARE NOT LIMITED TO)**:

- Administering medication that comes in the original packaging and is properly labeled.
- Maintaining program materials, equipment, program areas, supplies, etc.
- Maintaining petty cash (up to $20.00 per participant).

**6.** **Health and safety**:

- Ensure Health and Safety Program requirements are followed based on the Occupational Health and Safety Act
- Ensure an awareness of safety measures for the agency, yourself and the individuals receiving services
- Ensure compliance with the agency’s mandatory training requirements
